Care Initiatives is a nonprofit senior care provider based in West Des Moines, Iowa, offering skilled nursing, memory care, rehabilitation therapy, assisted and independent living and hospice services. Since 1989 it has grown to 43 skilled nursing locations, two assisted living sites, seven independent living sites and nine hospice offices across Iowa and eastern Nebraska, serving more than 2,500 residents and patients.
