Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier - protecting your time.\nWhat you will do:\nOwn the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes - not just individual visits.\n\nComplete comprehensive in-home assessments and deliver skilled PT visits.\n\nBuild and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.\n\nSet measurable goals, track objective progress, and adjust treatment quickly when patients stall.\n\nDrive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.\n\nManage treatment and outcomes directly, including PTA supervision where appropriate.\n\nCoach patients and caregivers with practical home programs that actually get done.\n\nDocument promptly and thoroughly - defensible, efficient, and consistent.\n\nWhy Adaptive:\nOperations that back you up: You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.\n\nClear productivity model with upside: You are paid per productivity point with an expectation of 28 points/week\n\nSchedule flexibility: We build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible.\n\nCutting-edge tech built for clinicians: Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes.\n\nMission-driven work: We are here to expand access to high-quality home health care, and we want therapists who take pride in raising the standard.\n\nMust-haves\nActive Colorado PT license (in good standing)\n\nStrong clinical judgment and comfort working independently in the field\n\nComfortable escalating changes in condition to RN/MD and documenting rationale clearly\n\nStrong communication and documentation\n\nReliable follow-through on orders, frequencies, and patient goals\n\nCurrent driver’s license and comfort with daily travel across assigned territory\n\nAble to carry/transport basic therapy equipment as needed\n\nCompetency in core home health PT scope\nSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations\n\nStrengthening, functional mobility, and home safety assessment/modification education\n\nPatient/caregiver coaching with practical home programs\n\nNice-to-haves\nPrior home health experience\n\nAxxess experience (or similar home health EMR)\n\nDeeper clinical experience in high-signal PT populations\n\nExperience supervising/mentoring PTAs and collaborating on care plans\n\nStrong organization and time management\n\nThe rest of the package\nFull-time target: ~28 productivity points/week;
