Our platform also includes an after-visit voice documentation option, allowing you to capture your notes afterward and leverage AI to draft your charts.\nBy streamlining documentation and reducing after-hours charting, our technology helps you spend less time on paperwork and more time doing what you do best, providing exceptional patient care.\nThe mission of this role\nDeliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.\nObjectives\nCarry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality.\n\nOwn your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.\n\nBe a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.\n\nSet the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.\n\nWhat you'll own\nDeliver skilled PTA visits in the home under the supervising PT’s plan of care.\n\nExecute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.\n\nSet/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.\n\nDrive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.\n\nCoach patients and caregivers with practical home programs that actually get done.\n\nDocument promptly and thoroughly - defensible, efficient, and consistent.\n\nWho you are / how you operate\nDedicated clinician: home health experience strongly preferred\n\nHigh ownership: you treat your caseload like your own panel and your pod like your own business.\n\nOpen mindset: Energized by the opportunity to shape the playbook and refine SOPs\n\nTech-forward: You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better\n\nTrust-builder: Patients, families, and referral partners believe you because you do what you say.\n\nRequirements\nActive Michigan PTA license (in good standing)\n\nReliable transportation, valid driver's license, auto insurance\n\nHome health experience strongly preferred\n\nAble to carry/transport basic therapy equipment as needed\n\nCompetency in core home health PTA scope\nSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations\n\nStrengthening, functional mobility, and home safety assessment/modification education\n\nPatient/caregiver coaching with practical home programs\n\nOur guarantee\nFull-time target: ~30 productivity points/week;
