Care is provided in accordance with ACH standards, regulatory requirements, and within the scope of practice for LPNs.\nResponsibilities:\nProvide age-appropriate nursing care under the supervision of a Registered Nurse (RN) or designated provider, applying basic knowledge of biological, physical, behavioral, social, and nursing sciences to meet the needs of individuals and groups.\nCollects and monitors patient data, including packaging and obtaining specimens and labs, contributing to assessments and promptly communicating abnormal findings or changes in condition to the appropriate provider.\nParticipates in developing and implementing patient care and discharge plans under RN direction, delivers nursing care and treatments as ordered, and assists with personal care and activities of daily living to maintain hygiene and nutrition.\nSafely administer prescribed medications, injections, and treatments while monitoring for adverse reactions, and ensure all providers orders are followed for patient treatment.\nProvides education to patients, families, and caregivers regarding health, treatments, and self-care.\nDocument all aspects of patient care in accordance with hospital policies and nursing care standards.\nAdheres to regulatory standards and ACH policies, maintains a safe and clean environment, and reports safety concerns as appropriate.\nWorks collaboratively as a member of the patient care team, engages in ongoing education and professional development, and performs additional duties to support efficient operations and quality patient care.\nOther duties as assigned.\nOther information:\nTechnical Expertise\nExcellent patient care, verbal, and written communication skills.\nAbility to work effectively with all levels within the organization.\nProficiency in MS Office Suite (Outlook, Excel, Word) or similar software.\nExperience with electronic medical records (e.g., Epic) preferred.\nPrevious experience in healthcare, home care, or physician office settings preferred.\nEducation and Experience\nEducation: Graduate from an accredited school of licensed practical nursing.\nLicensure: Current, valid LPN license in the state of Ohio.\nCertification: Basic Life Support (BLS) certification from the American Heart Association.
