This position offers the opportunity to provide high-quality cardiovascular care in a supportive, team-oriented environment while working closely with cardiologists and multidisciplinary care teams.\nThis role will primarily focus on outpatient follow-up visits and pre-operative cardiovascular evaluations, supporting continuity of care for patients with a wide range of cardiac conditions.\nPosition Highlights\nFull-time Monday-Friday 8am-4:30pm\nOutpatient clinic-based role\nAverage 15 patients per day once fully trained with 2 half days of admin time (8 patients on admin days)\nCollaborative practice with cardiologists, nurses, and ancillary support staff\nOpportunity to build expertise in outpatient cardiovascular management\nResponsibilities\nConduct outpatient follow-up visits for patients with cardiovascular conditions such as coronary artery disease, heart failure, arrhythmias, hypertension, and valvular heart disease\nPerform pre-operative cardiovascular evaluations and risk assessments for patients undergoing surgical procedures\nObtain detailed medical histories and perform focused cardiovascular examinations\nOrder and interpret diagnostic studies including laboratory tests, ECGs, and cardiac imaging as appropriate\nDevelop and implement treatment plans in collaboration with supervising cardiologists\nProvide medication management and patient education on cardiovascular disease prevention and management\nCoordinate care with primary care providers, surgeons, and other specialists\nDocument patient encounters and treatment plans accurately in the electronic health record\nParticipate in quality improvement initiatives and practice efficiency efforts\nIn addition, this role will focus on heart disease in women, with understanding of sex differences in cardiovascular disease, including prevention, atypical symptoms evaluation, microvascular disease, and managing women with hypertensive disorders of pregnancy\nEvaluate traditional risk factors alongside sex-specific and female-predominant risk enhancers, including adverse pregnancy outcomes, polycystic ovary syndrome, menopause transition and autoimmune disorders\nEducate female patients and their families on recognizing cardiac warning signs, understanding unique female-pattern risks, and adhering to prevention guidelines\nQualifications\nCurrent license and active registration as a Nurse Practitioner or Physician Assistant in New York State\nMaster’s degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program\nInterest in or experience with cardiovascular medicine preferred\nFamiliarity with electronic medical records; Epic experience preferred\nStrong communication and collaboration skills in a multidisciplinary clinical environment\nCommitment to patient-centered care and maintaining HIPAA confidentiality standards\nWe offer a highly competitive benefits package, including:\nCompetitive base salary range $116,035-$136,512 depending on years of licensure:Newly licensed: $116,035\n3 years of licensure: $122,861 \n8 years of licensure: $129,686\n12 years of licensure: $136,512\n\nComprehensive health insurance (medical, dental, vision)\nGenerous paid time off and holiday schedule\nAlbany Med retirement plan plus a 403(b)\nAnnual CME allowance and dedicated CME time\nOccurrence-based malpractice coverage\nEmployee wellness programs and mental health support\nAlbany Medical Center is the centerpiece of medicine, research, and medical education in New York’s Capital Region and is the area's largest private employer with more than 10,000 employees.\nAlbany Medical Center offers excellent career opportunities in a wide range of roles in both patient care and administration.
