Join our collaborative, mission-driven team and bring your skills to work that makes a real difference every day.\nAspire Living & Learning is currently seeking a Case Coordinator to be responsible for the management of cases under the supervision of a licensed clinical provider or department head. The Case Coordinator will work collaboratively with all internal and external team members to offer support where needed. \nSchedule: M-F 9a-5p\nLocation: Leominster, MA\nKey Responsibilities: \nThe Case Coordinator carries a caseload assigned by the supervisor, director, or department head. Individuals on the caseload may have varying degrees of need or oversight. Below outlines key aspects of the roles and responsibilities this position holds: \nImplementation and Training:\nDemonstrates the skills to work with a variety of individuals of varying ages and functioning level. \nDependent on service line, develops, oversees implementation of behavior, treatment, educational, or medication support plans, as applicable. \nAssists in the modeling and teaching of behavioral or mental health strategies to necessary stakeholders (e.g., staff, providers). \nTrains the managers, DSP leads, etc. on any plans, or individual specific training, when applicable. \nProgram Oversight:\nMonitors, verifies, and documents various levels of programmatic or clinical support or service provision. This may include collecting, aggregating, or reporting out on service specific data points or data collection procedures. Ensures accurate and timely reporting. \nAdministrative Tasks:\nCompletes all required documentation. \nMeets deadlines. \nEnsures relevant sections of electronic record is up to date and accurate. \nFacilitates or attends all team meetings. \nCommunication and Consultation:\nEffectively communicates in both verbal and written form with necessary stakeholders. \nAccepts feedback from supervisors to ensure the quality of program implementation. \nCommunicates and collaborates with both internal and external team members and stakeholders. \nMaintains ethical and professional boundaries and guidelines. \nWorks cooperatively and collaboratively with all necessary stakeholders. \nCultivate a positive and productive workplace atmosphere to enhance team morale and productivity. \nParticipate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role. \nConduct training as required and assigned. \nPerform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation. \nAccept reassignment as required. \nQualifications:\nHigh school diploma or GED required, bachelor's preferred \nMust meet the Registered Behavior Technician requirements \nExcellent interpersonal, organizational, and communication skills \nComputer skills \nMust have satisfactory background checks \nValid driver’s license and safe driving record \nEmployees at Aspire enjoy working with a collaborative and dedicated team that values a culture of belonging and care.
