Through practical and emotional support, seamless care coordination, and collaboration with local services, this role ensures tailored, proactive solutions that enhance quality of life and prevent crises.\nThis role will be based in our first Housing with Support Scheme, Richmond Place, located in Inchicore, Dublin 8. This is an onsite role.\nReporting Relationship\nYou will report directly to the Housing with Support Scheme Manager.\nResponsibilities:\nSupport coordination, devising personalised support plans to address challenges and find solutions to enhance the older persons quality of life.\nBuild and sustain strong partnerships with healthcare, housing, social care, and community-based service providers, serving as the central communication hub to ensure integrated care and high-quality support delivery.\nConduct comprehensive assessments to identify and address the needs of Older People, resolving challenges and mitigating risks, while regularly monitoring and reviewing care plans to ensure they remain aligned with individual needs.\nMonitor and manage key performance indicators (KPIs), striving to achieve SMART targets that maximize the reach and impact of support services for older people.\nProactively advocate for older people and identify and address potential issues through collaborative problem-solving.\nEncourage multidisciplinary teamwork to identify and address potential challenges early.\nLiaise with the older person’s healthcare provider, consultants, GPs and primary care teams to report concerns including health, functional, or cognitive decline.\nRecord keeping and report writing on ALONE’s CRM (Customer Relationship Management) database.\nImplement and manage the distribution, installation, and responsiveness to technology that enhance older people's well-being and safety within their homes. \nEnsure the safety and well-being of Older People by adhering to fire safety protocols and Health and Safety guidelines, monitoring and addressing potential risks in line with policy and procedure.\nTake appropriate action in the event of emergencies and critical incidents, following processes and procedures as necessary.\nAny other duties within the general requirements of the above that may be assigned\nEssential Skills and Experience:\nPatient Handling; Manual Handling; Basic First Aid desirable but not essential.\nQQI Level 5 in Care of the Older Person qualification is essential \nMinimum 1-2 year experience providing person-centered care and/or support. \nDemonstrate a keen interest in working with Older People and be committed to ALONE’s vision and mission, including ensuring that older people can age happily and securely at home and are strongly connected to their local communities.\nAbility to maintain the highest standards of confidentiality, discretion, and respect.\nExcellent communication skills (both written and verbal), and the ability to network and cooperate well with individuals, groups, and stakeholders, is essential.\nStrong problem-solving and analytical skills, including the ability to adapt your practice to meet the evolving needs of Older People. \nStrong administrative skills.\nGood computer literacy, with experience of using case management systems, is desirable.\nExcellent organisational skills, report writing, solution-focused skills and multi-tasking skills are desirable.\nExcellent active listening skills.\nA passion for providing a high quality of service is essential.\nExtra Benefits:\nSalary €37,000\nAnnual Salary Reviews\nPension Scheme -
