Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms.\nResponsibilities\nPrimary Job Functions:\nDaily responsibilities:\nAssess consumers for areas in which they are experiencing deficits as determined by the DLA-20\nInstruct patients on how to improve their abilities in the deficit areas.\nDemonstrate and assist consumer in performing their tasks to evaluate comprehension.\nEducate patient on how to continue to implement the skills they have acquired.\nMaintain established institutional policies and procedures, objectives, quality assurance program, and safety, environmental and infection control standards.\nDemonstrate the ability to recognize the elements of a crisis and knows how to deescalate or resolve the situation.\nCollaborates with supervisor to ensure all steps needed to engage patients have been met and patient is set up for success.\nCollaborates with treatment team members and regularly provides feedback on the patient’s status.\nSupervision and Consultation\nSeeks clinical supervision and consultation as needed.\nAccepts and employs suggestions for improvement.\nClinical Record Keeping\nDocuments in a timely fashion per AltaPointe policy, within 24 hours of contact with patients.\nDocuments appropriate and complete provision of services in each patient’s chart\nCourteous and respectful attitudes towards consumers, visitors and co-workers\nTreats patients with care, dignity and respect\nRespects patient’s privacy and confidentiality\nIs sensitive to the patient’s needs, expectations and individual differences\nAdministrative and Other Related Duties as Assigned:\nFollows AltaPointe policies and procedures\nAttends required in-service training, scheduled meetings and other workshops \nPhysical Requirements of the Job:\nThere is constant standing\nSitting is frequent by nature and up to 5-10 minutes; however, extended sitting could be up to 1.5 to 3 hours for documentation.\nWalking for extended periods of time.\nSquatting and/or lunging frequently\nFine manipulation for writing and handling paperwork\nPushing and/or pulling force up to 1-5 pounds force, horizontally, to open file cabinets and place binders on bookshelves.\nLifting various ranges from the floor to overhead with weights up to 5-15 pounds frequently, 30-40 pounds from floor occasionally, 50 pounds from floor to waist 1-2 times per day\nAny other duties as assigned by supervisor\nQualifications\nMinimum Qualifications:\nPossess a High School Diploma or equivalent.
