We are seeking a Community Care Coordinator to ensure a smooth patient experience by managing scheduling, coordinating provider availability, and delivering exceptional customer service.\nKey Responsibilities\nWorks directly with a facility dental liaison to ensure successful on-site dental care and oral health care screening visits\nPrepare the clinical provider schedules for on-site care visits\nReport, document, and address customer service inquiries from patients, patient representatives, and facility staff members\nCoordinate monthly visits to our contracted on-site care facilities for dental services and oral health screenings\nEnter monthly provider schedules into Open Dental and distribute to appropriate staff\nUpdate facility records as needed\nConfirm patient appointments, with family members and/or the facility staff\nScan and enter new and/or updated patient information\nEnter and verify insurance plans, and document changes and updates accordingly\nPerform pre and post quality assurance for appointments and send insurance claims\nSend and follow up on treatment plans sent to families and update status accordingly\nSend and follow up on insurance pre-authorizations and update status accordingly\nScan and process patient treatment information including fax correspondences\nDiscuss appointment and account information with patients and their responsible parties\nWhy Join Us?\nMeaningful Work -
