highest possible rating awarded by the publication. \nConceives and creates care plans based on a comprehensive resident assessment that is accomplished through resident and family interviews, physical assessment, observation, and input from the care team.\nSchedules, within the established time frame, PPS/MDS assessments for residents admitted to the nursing care center.\nLeads the assessment coordination process to ensure accurate and timely completion of all the nursing sections of the MDS assessments.\nCompiles data for resident insurance updates for residents admitted with PPO and managed care payer sources.\nCommunicates effectively with the Director of Therapy and Insurance Case Managers to maximize level of reimbursement.\nLeads the MDS completion process by ensuring accurate and timely completion of all sections by responsible disciplines.\n Must be a Registered Nurse currently licensed in Maryland, with at least 2 years' experience in a similar role.\nTwo years of Long-Term Care experience.\nRAC-CT preferred but not required. \nExperience with PointClickCare preferred but not required. \nMust be able to follow and give accurate written and oral instructions and possess visual acuity to effectively prepare and read reports. \nSupervisory and leadership skills with the ability to communicate effectively with staff, residents, and visitors. \nAbility to coordinate and organize own work and the work of others to effectively meet multiple job responsibilities and deadlines. \nMust have proficient computer skills to ensure accurate data entry/Microsoft
