We take a responsible approach to creating environments that allow us to do what we do best - provide healthcare and wellness.\nSummary\nThe Community Connections Referral Program Supervisor oversees the daily operations of the CCS program, ensuring high-quality service delivery and strengthened family functioning. This role provides leadership, supervision, guidance, and data oversight to staff working directly with families, ensuring timely assessments, effective linkage to community resources, and compliance with contractual expectations.\nEssential Duties and Responsibilities\nProvide daily oversight of the CCCS service team to ensure high-quality service delivery and effective family support.\n\nAssist staff with assessments, case planning, and responses according to agency policy and procedures.\n\nSupervise a team of five to six staff, providing weekly supervision and case consultation.\n\nEnter supervision notes into SACWIS within 48 hours.\n\nGuide and coach staff to strengthen service provision and maintain compliance.\n\nMonitor staff performance and engage in solution-focused responses when issues arise.\n\nImplement disciplinary measures when needed, following agency protocols.\n\nFacilitate team meetings, staffing discussions, and task delegation.\n\nDevelop systems to track weekly documentation, assessments, and case progress.\n\nMonitor team performance against contractual benchmarks and program outcomes.\n\nAnalyze and respond to dashboard metrics, internal tracking systems, and performance data.\n\nEnsure timely monthly billing to the Department.\n\nSupport staff professional development and provide ongoing education.\n\nAssist staff in connecting families with appropriate community resources.\n\nBe available for crisis needs outside standard business hours when necessary.\n\nQualifications\nEducation and Experience:\nMaster of Social Work (MSW) or related Master’s degree required.\n\nTwo years of full-time supervisory experience in a social work setting required.\n\nExperience working with youth, families, and diverse communities.\n\nKnowledge, Skills, & Abilities:\nStrong understanding of child welfare systems, trauma, mental health, substance abuse, and family systems.\n\nKnowledge of community resources supporting children and families.\n\nProficiency with Microsoft Office and SACWIS; ability to learn new systems quickly.\n\nStrong written and verbal communication skills.\n\nExcellent time management, organizational, and leadership abilities.\n\nAbility to manage multiple competing priorities and respond to crises as needed.\n\nCapacity for reflective practice, problem-solving, and data-driven decision making.\n\nCertificates & Licenses\nMust possess or become certified in IDCFS CWEL, CERAP, and CANS (agency supports training).\n\nValid Illinois driver’s license and current automobile insurance.\n\nReliable personal vehicle required for transporting clients.\n\nMust obtain and maintain First Aid and CPR certification within 3 months of hire.\n\nTrauma-Informed Care Expectations\nAll employees are expected to use a trauma-informed approach based on the 5 S’s:\nSafety - Creating physical and emotional safety for families and staff.\n\nSelf-Regulation - Managing one’s own emotions during challenging interactions.\n\nSupportiveness - Maintaining compassionate, respectful client relationships.\n\nStrengths - Identifying and building upon strengths in every family.\n\nSelf-Care - Practicing strategies to reduce secondary trauma and burnout.\n\nWorking Conditions & Physical Demands\nRequires regular driving, home visits, and community-based work.\n\nMust be able to lift up to 50 lbs occasionally.\n\nExposure to various environments, including outdoor weather and community settings.\n\nModerate noise level typical of office and field environments.\n\nCompensation & Benefits\nSalary: $72,125- $74,788 (based on experience)\n\nHealth, Dental, and Vision Insurance\n\nDisability & Life Insurance\n\nPaid Vacation, Sick Days, and Holidays\n\nRetirement Plan\n\nEmployee Assistance Program (EAP)\n\nDiscounts on services such as cell phones, restaurants, and more\n\nEqual Opportunity Statement\nAunt Martha’s Health & Wellness is an Equal Opportunity Employer.
