We're building the operating layer for decentralized care.\n\nWhat this role is really about : \n\nYou'll be shaping backend systems used directly by clinicians and care teams - where scheduling, coordination, and data exchange need to work reliably across fragmented systems. This isn't just about writing code - you'll also shape how backend engineering is done here.\n\nYou'll : \n\n- Set the technical direction\n\n- Raise the bar on system design and execution\n\n- Help other engineers grow into strong system thinkers\n\nIf you enjoy owning both systems and the people building them, this will feel like the right scope.\n\nWhat you'll work on : \n\nBackend systems that hold up in the real world : \n\n- Design and evolve backend services using Node.js and TypeScript\n\n- Build REST APIs and GraphQL endpoints powering web and mobile applications\n\n- Model and manage data using PostgreSQL and related tools\n\n- Work with event-driven systems for coordination, notifications, and integrations\n\nAI as part of the system : \n\n- Integrate AI into backend workflows - automation, orchestration, and decision systems\n\n- Work with LLMs, AI pipelines, and intelligent data flows in production\n\n- Think through real trade-offs : latency, reliability, cost, and safety\n\nData that drives real-time decisions : \n\n- Design data flows and pipelines that support operational workflows\n\n- Work with tools like Airbyte, dbt, Airflow / AWS Glue where needed\n\n- Enable systems to react in real time\n\nSystems that don't get to go down : \n\n- Run and scale systems on AWS (ECS, EC2, S3, RDS, Lambda)\n\n- Own CI/CD, observability, and production reliability\n\n- Build for environments where failures have real-world consequences\n\nInteroperability : \n\n- Work with HL7 / FHIR and EMR integrations\n\n- Handle real-world healthcare data and constraints\n\n- Design systems that make disconnected platforms actually work together\n\nLeadership & Ownership : \n\n- Set direction for backend architecture and engineering practices\n\n- Review designs and code with a focus on long-term quality and scalability\n\n- Mentor engineers and help them grow in system design and ownership\n\n- Drive clarity in ambiguous problem spaces and align teams around solutions\n\n- Partner closely with product, frontend, mobile, and integrations teams\n\n- Take ownership of outcomes\n\nWhat we're looking for : \n\n- Strong hands-on experience with Node.js, TypeScript, and distributed systems\n\n- Experience designing APIs (REST / GraphQL) and working with PostgreSQL\n\n- Comfortable working with event-driven architectures\n\n- Experience with AWS and production-grade infrastructure\n\n- Ability to reason about systems end-to-end\n\n- Experience leading projects or mentoring engineers\n\n- Exposure to healthcare (HL7/FHIR) is helpful but
