Performs data collection, quality review and communication of data to improve performance and enhance outcomes.\nThe following is intended to describe the general nature and level of work being performed by individuals assigned to this job by listing the major components for this position (i.e., key responsibilities and essential duties). Additionally, other duties may be assigned at the discretion of leadership to meet business, customer, and/or patient needs.\nPerforms accurate and timely data collection\nDemonstrates thorough knowledge of indicators, guidelines and regulations related to database activity.\nAbstracts medical records in an accurate and timely manner.\nReviews medical records for clinical quality measures and other performance indicators\nDevelops systems and processes for timely and accurate data collection.\nDevelops and maintains accurate data retrieval systems.\nSubmits data to databases, as well as insurance companies, ensuring accurate and timely submission.\nEncourages and facilitates team participation in decision making process by providing accurate data to facilitate decisions.\nAnalyzes data in a timely and accurate manner:\nPerforms secondary review and analysis of outliers and confirms outlier status prior to reporting.\nPerforms statistical compilation and reporting as requested.\nIdentifies trends or potential problems in results and communicates to the Director of Quality and other internal and external entities.\nAssists with locating appropriate benchmarks.\nAnalyzes benchmarked data and develops reports for internal and external customers identifying practice patterns, and other areas of concern, with recommendations for improvement as appropriate.\nMakes sound judgments based on factual information.\nCommunicates with director to resolve problems/concerns, making suggestions to improve outcome processes to enhance team efficiency and productivity.\nParticipates as a team member as requested.\nFacilitates meetings and teams as requested.\nUtilizes Process Excellence methodologies such as DMAIC, LEAN, Six Sigma, Change Management, PDCA and FOCUS PDCA\nAttends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards, and national patient safety goals.\nManages difficult or emotional customer situations\nResponds promptly to customer needs\nSolicits customer feedback to improve service\nResponds to requests for service and assistance\nRequirements:\nBachelors of Science in Nursing (BSN) required;
