$49,763.00 - $69,668.00The Triage Coordinator serves as the primary referral intake specialist for the Center for Donation and Transplant (CDT), working remotely on a per diem basis with variable shifts hours. The Triage Coordinator responds to hospital-vented referrals via direct phone communication, completes donor suitability evaluations through electronic medical record (EMR) review, and gathers medical history via statewide health platforms. All data is relayed to the Administrator on Call (AOC) to complete suitability screening and dispatch additional resources as required. The Triage Coordinator is also responsible for organ import offer screening to support local transplant centers. The coordinator communicates organ offer data to surgeons for deferral or acceptance and serves as a point of communication between the offering Organ Procurement Organization (OPO) and the accepting transplant center

Essential Duties and Responsibilities

  • Perform initial screening of referrals by communicating clinical information to appropriate subject matter experts (AOC, Medical Director, CDT Leadership, team members).
  • Complete timely documentation of medical screening via CDT EMR data entry, following referral intake protocols and policies.
  • Evaluate the medical suitability of potential organ donors using clinical information from EMRs, health platforms and verbal clinician reports.
  • Communicate clinical information to team members and stakeholders using multiple modalities (phone, email, MS Teams, etc.).
  • Interpret comprehensive medical chart data, including but not limited to laboratory values, imaging reports, history and physical notes, progress reports, and social work reports.
  • Collaborate with the AOC, CDT clinical team, and hospital care team to develop an action plan that maintains the opportunity for donation.
  • Close out referrals and communicate outcomes to the hospital care team as directed by the AOC.
  • Review organ offers for the local transplant centers using up-to-date screening criteria.
  • Access DonorNet to screen organ offers with the on-call transplant surgeon.
  • Document appropriate acceptance or decline codes in DonorNet.
  • Follow OPTN policies for all organ offers, ensuring timely responses to the host OPO and receiving transplant center.
  • Serve as a communication conduit between the host OPO, accepting transplant center, and necessary stakeholders.
  • Document outcomes of all accepted organs in the CDT EMR system.
  • Actively participate in logistics problem-solving to maximize opportunities for the local transplant center.
  • Maintain healthy, open, honest, and professional relationships with CDT team members, hospital staff, donor families, medical examiners/coroners, and other professionals encountered in the performance of duties.
  • Inform the supervisor or director of emergencies and unusual interactions or conflicts involving those we serve.
  • Maintain confidentiality when interacting with patients, families, staff, and the public, in accordance with CDT policy and applicable regulations.
  • Confirm hospital conformance with regulations including referral timeliness and policy standards.
  • Manage time effectively to allow for preparation of educational programs and other departmental activities.
  • Accept responsibility for completing assigned work; seek assistance from co-workers when needed.
  • Remain compliant with department policy regarding scheduled work time (e.g., begin work on time; inform the supervisor of changes or absences).
  • Document, notify, and communicate reportable events to the Director of Clinical Operations.
  • Follow and meet CDT staffing requirements based on per diem scheduling policies and/or guidelines.

Qualifications

  • Associate's Degree in a healthcare-related field - required
  • Bachelor's Degree in a healthcare-related field - preferred
  • 1-3 years of prior or current organ procurement or related experience - preferred
  • 1-3 years with on-call scheduling - preferred
  • Familiarity with OPTN policies and procedures, including organ allocation (preferred).
  • Previous data entry and computer experience (preferred).
  • Ability to work extended hours.
  • Ability to keep accurate records, perform calculations, follow guidelines, draft policies, and conduct technical training.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in NYS Upon Hire - preferred
  • RRT - Registered Respiratory Therapist Upon Hire - preferred
  • PARAMEDIC - Paramedic - Licensed Upon Hire - preferred

Equivalent combination of relevant education and experience may be substituted as appropriate.
