In 2022, Bluestone Accountable Care Organization (ACO) was the best performing ACO in the country as measured by the overall savings per Medicare beneficiary. \nPosition Overview:\nThe Care Coordination Data Entry Specialist works independently assisting the Operations Teams with delegated tasks as needed such as enrollment side duties, outreach to members, mailing documents, and data entry into state systems. This position will be supporting members from different health plans and products. \nSchedule: Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays. \nLocation: Include description of radius as necessary\nSalary: $24.00 - $25.00 per hour. Salary will be commensurate with experience. \nResponsibilities:\nAssist with mailing out time sensitive documents to members\nComplete outreach calls to members and or legal guardians to follow up on necessary documentation needed for support plans \nWork efficiently and accurately in the MnChoices system\nLooking up members in MNITs for eligibility of waivers and insurance information \nStay up to date with health plan changes and requirements to stay in compliance\nParticipate in regular team meetings\nParticipate in regular health plan meetings\nOther duties as assigned by the Operations Manager\nQualifications:\nEducation/Certification/Experience\nAssociate’s degree in Human Services, Public Health Community Service, etc.- Preferred\nIdeal candidate will have 3+ years experience working in a related field (hospital, HUC, customer service, CMA or other related field)\nIntermediate-level computer proficiency with email, fax, word processing, spreadsheets, and data bases.
