The Designated Coordinator works closely with individuals served, support teams, and direct support staff to ensure services are delivered according to each person’s Coordinated Service and Support Plan, while helping program teams provide consistent, individualized support.\nWhat You’ll Do\nProgram & Client Services\nCoordinate services for individuals receiving community-based supports\n\nConduct client intakes, admissions, assessments, and service planning activities\n\nDevelop, implement, review, and update individualized service plans\n\nAttend support team meetings and serve as a liaison with guardians, case managers, and other providers\n\nMaintain required client service agreements, reports, monthly summaries, and other program documentation\n\nSupport incident response and required reporting\n\nHelp ensure services remain person-centered and aligned with individual goals\n\nStaff Leadership & Development\nProvide supervision and support to IHS Specialists and other assigned program staff\n\nMaintain staffing patterns based on client needs\n\nConduct regular 1:1 meetings, performance reviews, coaching, and performance improvement as necessary\n\nTrain staff on client-specific supports, person-centered practices, shadow shifts, and service requirements\n\nSupport staff competency so employees are prepared to work independently with individuals receiving services\n\nFacilitate communication between program staff and the management team\n\n245D Compliance & Quality\nEnsure services are delivered in accordance with Minnesota 245D requirements\n\nMonitor completion of required annual staff training\n\nSupport compliance with service planning and intensive support service requirements\n\nConduct client file, documentation, and assessment audits\n\nMaintain required program documentation and compliance deadlines\n\nIdentify opportunities to improve service quality and address compliance concerns proactively\n\nProgram Operations\nComplete weekly dashboard metrics and Designated Coordinator checklists\n\nParticipate in management meetings and organizational planning\n\nSupport development and maintenance of program policies and procedures\n\nStay informed of regulatory and legislative changes affecting services\n\nDevelop relationships with counties, regions, and community partners to support program development\n\nParticipate in BrightPath’s on-call rotation and 24-hour emergency assistance program\n\nDirect Service\nThis is a hands-on leadership position. The Designated Coordinator is expected to provide direct care based on service line needs and scheduling.\nRequired Qualifications\nTo meet Minnesota 245D Designated Coordinator requirements, candidates must qualify through one of the following pathways:\nBachelor’s degree in a human services-related field and at least one year of full-time direct care experience serving people with disabilities or adults age 65 or older;\n\nAssociate degree in a human services-related field and at least two years of qualifying full-time direct care experience;\n\nDiploma in a human services-related field from an accredited postsecondary institution and at least three years of qualifying full-time direct care experience; or\n\nA combination of human services education/training and qualifying supervised experience meeting Minnesota 245D requirements.\n\nAdditional Experience & Skills\nSupervisory and/or leadership experience in a social service organization\n\nExperience working with Minnesota 245D licensing requirements\n\nStrong written and verbal communication skills\n\nAbility to organize, prioritize, and independently manage competing responsibilities\n\nSound judgment and the ability to make decisions with appropriate discretion\n\nAbility to adapt effectively as client, staffing, and program needs change\n\nWillingness to learn and apply new approaches and practices\n\nValid driver’s license, reliable transportation, current auto insurance, and ability to meet BrightPath driving requirements\n\nBenefits\nBrightPath offers:\nIndividual Coverage Health Reimbursement Arrangement (ICHRA) for medical coverage\n\nDental and vision insurance through Mutual of Omaha\n\n401(k) with a 3.5% company match\n\nPaid time off\n\nPaid holidays\n\nEmployee Assistance Program\n\nOngoing professional development opportunities\n\nAbout BrightPath\nBrightPath was established to enable the freedom to pursue joy by providing quality services for individuals with disabilities.
