We strive to break the cycle of homelessness with personalized, wraparound support, empowering individuals and families to rebuild their lives with stability, dignity, and hope.\nPosition Summary\nThe Brook Supportive Housing Program is seeking a Clinical Coordinator to join an established multidisciplinary team. The Brook permanently houses 189 residents, the majority are formerly street homeless and have serious mental illness, chronic medical conditions (including HIV/AIDS) and substance misuse challenges. Reporting to the Program Director, the Clinical Coordinator is a vital member of our team. The position involves a unique blend of staff management and direct service provision. LMSW is required (MSWs are welcome to apply- however must obtain license within 3-6 months of hire). Clinical Coordinators provide psychosocial support, medication monitoring, financial management, crisis intervention, treatment planning and other supportive case management services and oversight to single adults in a community environment. With opportunities to further develop the clinical skills required to effectively manage the case management needs of a triply diagnosed population.\nIdeal candidates can demonstrate the ability to work in partnership with our residents from a strengths-based perspective, overcoming stigma, and navigating complex systems. Additionally, the ideal candidate can adapt to an active environment, is exceptional with time management, has strong writing and communications skills, exhibits excellent attention to detail and remains a team player. Opportunities for group work are also available as are LCSW qualifying hours.\nQualifications\nLMSW (Licensed Master Social Work)\nMinimum of two years' experience working with homeless populations \nExperience with providing behavioral health services\nEssential Duties and Responsibilities\nContribute towards the ongoing development and quality of services provided \nAdhere to the agency and funder policies, principles, and procedures (DOHMH, HASA and DHS) \nHold ongoing clinical and task supervision with social services staff. \nSupervise documentation of progress notes and ongoing assessments in AWARDS \nMaintain physical case records and statistics for program to analyze trends and improve service delivery \nEnsure all programs meet contractual goals and reporting requirements \nAdminister personnel policies and procedures under the guidance of the program director \nAssist the program director in recruiting and training direct service staff \nParticipate in, and encourage staff to become involved in agency events \nAct as Liaison with medical clinic staff on and off-site as needed \nCoordinate clinical services for Residents including discharge planning \nProvide crisis management and on-going clinical assessments \nProvide supportive counseling and behavioral health services\nRemain on call after hours and on weekends to assist with resident medical and psychiatric incidents \nAssist with financial management for residents enrolled in our representative payee program\nComplete Medication Monitoring Training within 3-6 months of hire\nPerform additional duties as assigned\nProgram Specific Skills\nStrong writing and oral communication skills\nExcellent organizational, teambuilding, and leadership skills\nDemonstrated skills to work within a team model and provide team leadership in team meetings\nExperience supervising case management staff preferred\nStrong presentation skills and ability to represent the agency at functions and meetings\nAbility to handle complex challenges and balance multiple demands on time and attention \nCommunity minded, patient, creative, flexible, and compassionate \nDemonstrated proficiency with MS Office, electronic databases and prolonged use of a computer\nSkills/Abilities\nAbility to occasionally lift and/or move up to 10 pounds\nAbility to stand, walk, or sit for long periods of time\nAbility to conduct home visits and/or fieldwork\nAbility to bend and retrieve objects and/or documents\nAbility to travel in the boroughs of New York City and its adjacent counties via public transportation\nAbility to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work\nBronxWorks is an equal opportunity employer committed to an inclusive workplace.
