Position: Licensed Vocational Nurse (LVN) 

Reports To: Team Leader

About the Role

We’re looking for an LVN who is compassionate, reliable, and passionate about patient care. In this role, you’ll provide direct care under the guidance of a Registered Nurse, follow the plan of care, and work closely with our interdisciplinary team to ensure each patient receives the support they need.

What You’ll Do

  • Provide hands-on care to patients as allowed by the State Nurse Practice Act

  • Carry out care plans based on assessments and patient needs

  • Document visits clearly and on time

  • Help educate patients and families so they feel informed and supported

  • Communicate patient needs to the wider hospice team (aides, therapists, social workers, dietitians, supervisors, etc.)

  • Use equipment and resources safely and responsibly

  • Support pain and symptom management, and monitor how patients respond

  • Participate in training, development, and quality initiatives

  • Assist with other tasks as requested by the RN

What We’re Looking For

  • Graduate of an accredited LVN/LPN program

  • At least three years of nursing experience (hospice, community health, or med/surg is a plus)

  • Active LVN/LPN license in the state

  • Strong communication and organizational skills

  • Valid driver’s license and a reliable, insured vehicle

  • Current CPR certification

  • A genuine understanding of the hospice philosophy and the needs of the terminally ill

Hourly range for posted region

$35—$45 USD
