Using your care advocacy skills and experience, you’ll offer guidance and support to patients and their families-helping them access care, clarify provider instructions, connect with community resources, and reduce barriers to a healthier, more independent life.\nThis is a remote, 1099 (contractor) role. You may choose part-time or full-time engagement.\n NOTE: Minimum 20 hours of availability per week (Monday-Sunday), including at least 75% during standard business hours (Monday-Friday, 9:00 AM-5:00 PM).\nWhat You’ll Do\nDevelop trusting relationships with patients, grounded in listening, respect, and clarity.\n\nAssess the whole person - medical, social, and environmental factors - to pinpoint barriers and needs.\n\nCreate individualized care plans that integrate medical, social, and behavioral supports.\n\nSupport patients across a range of needs, including:\n\nConnecting to local services (transportation, meal delivery, home care, social supports)\n\nAnalyzing medical bills and helping with claim appeals or explanations\n\nIdentifying medication cost savings, co-pay assistance, or formulary options\n\nLinking to social support networks (caregivers, community programs, peer groups)\n\nOrganizing and maintaining medical records, care histories, and provider communication\n\nCoaching and supporting patients on best practices for managing conditions and navigating care\n\nDocument interactions, observations, and patient progress in Baba’s systems with clarity and timeliness.\n\nCollaborate with clinical, product, and operations teams to optimize workflows, escalate complex issues, and refine the patient experience.\n\nProvide feedback on platform features, tools, and processes to improve Baba’s care copilot capabilities.\n\nWhat You Bring\nAssociate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or a related healthcare field OR Certificate/Diploma in Practical Nursing (LVN/LPN).\n\nActive RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred.\n\n2+ years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work, preferably supporting older adults or Medicare populations.\n\nSolid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals.\n\nDeep understanding of Social Determinants of Health (SDOH) and experience working with diverse and underserved populations.\n\nEmpathy, patience, and strong communication skills- you can explain complex things simply.\n\nComfort working with digital tools, EMRs / care-management software, and adapting to new tech.\n\nCommitment to continuous improvement- you’ll raise issues, suggest solutions, and help refine Baba’s model.\n\nOrganized, proactive, and able to juggle multiple priorities without losing
