The Intake Coordinator plays an essential role in converting new referrals, guiding clients and families through the onboarding process, and maintaining strong relationships with referral sources. This position helps ensure each new client receives a smooth, timely, and supportive intake experience from the first point of contact through the transition to care.\nSchedule: Tuesday - Saturday 8:30AM - 5:30PM\nResponsibilities\nReferral Conversion\nManage new referrals in accordance with department standards and company guidelines.\nConduct timely outreach to prospective clients, families, and referral sources.\nMaintain accurate and up-to-date client status information.\nPartner with referral sources to obtain updated client information and support successful conversion.\nClient Onboarding\nEducate clients and families on available services to support a successful transition to care.\nDocument intake activities accurately and as directed.\nCommunicate relevant client information to the staffing team to support an appropriate caregiver match.\nHelp ensure clients and families feel informed, supported, and confident throughout the onboarding process.\nRelationship Building\nRespond to referral emails and inquiries in a timely and professional manner.\nProvide referral sources with timely updates regarding client and family outreach.\nMaintain referral source portals with current and accurate information.\nBuild and maintain positive working relationships with referral sources.\nQualifications\nQualifications\nAssociate's degree or equivalent combination of relevant education and experience.\nStrong verbal and written communication skills.\nAbility to work effectively in a fast-paced environment.\nComfortable managing a high volume of calls and client interactions.\nStrong attention to detail and organizational skills.\nFluency in both English and Spanish.\nJob Requirements\nMust be at least 18 years of age.\nUS Work Authorization.\nAbility to pass
