CAMH is currently seeking (6) full-time, Permanent Service Coordinator to provide client-centered\nintake, screening, and service coordination across the mental health and addictions (MHA)\nsystem.\nCentre for Addiction and Mental Health (CAMH) has been selected as the Mental Health and Addictions\nHub to establish a centralized access point for outpatient hospital and community mental health and\naddictions service in the Toronto Region. This Hub is responsible for coordinating timely access to\nservices and will play a key role in transforming access to care.\n Facilitate access to care by reviewing referrals, engaging with clients and families, conducting\nbrief screening, and connecting clients to appropriate services.\nReview incoming referrals and determine eligibility for MHA services, triaging referrals to\nappropriate priority levels and service type and matching to the most appropriate service\nprovider utilizing various referral management system.\nContribute to process improvement, stakeholder engagement and data collection to ensure the\nprovincial model is implemented with efficiency and equity.\nLiaise with individuals, referral sources and service providers to ensure timely and appropriate\naccess to care.\nParticipate in regional exchange sessions and quality improvement activities to enhance service\ndelivery.\nFoster positive relationships with referral sources, community partners, and service providers\nacross the Toronto Region\nMaintain confidentiality of patient information in alignment with professional and\norganizational requirements\nSupport a workplace that embraces diversity, encourages teamwork and complies with all\napplicable regulatory and legislative requirements.\nThis position is located at 1051 Queen Street West. The position will require Monday to Friday\nday and evening shifts (0800-2000).\nBachelor’s degree in Social Work, Psychology, Health Sciences, Behavioural Sciences, Nursing,\nor, related field is required\nA Regulated Healthcare Professional (Social Work, Occupational Therapy, Nursing) is preferred.\nMust be a member in good standing with the respective regulatory body.\n3-5 years’ experience in the mental health and/or addictions sector providing direct patient/client care, including intake, service navigation, or case management.\nExperience in coordinated access for mental health and addiction services preferred\nAbility to efficiently synthesize information from a variety of sources (referral form, patient and\nfamily interview, Connecting Ontario, previous medical records) to determine appropriate care\nneed and match individuals to the right care.\nGood clinical judgement to identify urgency of care need and respond using established\nprotocols (eg. 911 calls, transfer to crisis service).\nExcellent organizational, interpersonal, and written communication skills and the ability to work\neffectively in a fast-paced clinical environment are essential.\nProficiency in Microsoft office, Hospital Medical Record Systems and electronic communication\ntools required\nFamiliarity with referral management system an asset\nWork effectively as a team member in a dynamic interprofessional environment and be a self\u0002starter.\nExperience working with clients, staff, and organizations with diverse backgrounds is required.\nBilingualism (French/English) or proficiency in a second language is an asset.\nIf you are interested in this position, please ensure that you apply before 5pm EST of the closing date.
