Job Description: The Assessor Coordinator - Degree provides full case management services under managerial direction within an interdisciplinary team. This role is patient centered, flexible, cost effective, and quality driven, focusing on advocating for clients and coordinating resources based on assessed needs. Working in Primary Health Care, the position serves as an entry point for accessing services such as community, home, long term, and acute care. Key duties include intake, engagement, care planning, monitoring, evaluation, and discharge. The role collaborates with healthcare providers, families, caregivers, and agencies to assess needs, coordinate services, and ensure timely, appropriate care delivery.

Education
- Bachelor Degree in a related Health Science or Social Science discipline
Knowledge and Abilities
- Ability to manage a high demand caseload and other work assignments.
- Ability to facilitate applications/participate in integrative case management
- Ability to appropriately intervene/attain intervention in emergency situations
- Demonstrated knowledge and ability for competent case management practice
