This position serves as a key liaison between patients, physicians, clinical staff, and program management, facilitating communication and coordination throughout the patient care process.\nEssential Responsibilities\nOversee daily patient flow and coordinate the movement of patients through the program to support timely and efficient care.\nServe as a primary liaison between patients, physicians, clinical staff, and the program manager.\nFacilitate communication regarding patient needs, appointments, scheduling, and program-related concerns.\nSupport physicians and clinical staff with the coordination of daily program activities and patient needs.\nAssist the program manager with day-to-day operations, workflow, scheduling, and administrative functions.\nMonitor program activities and identify operational issues that may affect patient flow or the patient experience.\nCoordinate with internal departments and external providers as needed to ensure continuity and efficiency of services.\nMaintain accurate patient and program documentation in accordance with organizational policies and regulatory requirements.\nProvide patients and families with information regarding program services, processes, and resources.\nAssist with resolving patient concerns and escalate clinical or operational issues appropriately.\nCollaborate with the clinical team to ensure patient needs are communicated and addressed in a timely manner.\nSupport the implementation of program policies, procedures, quality initiatives, and process improvements.\nMaintain confidentiality and comply with HIPAA, organizational policies, and applicable healthcare regulations.\nWhat We're Looking For\nCurrent, active New Jersey Licensed Practical Nurse (LPN) license required.\nGraduate of an accredited practical nursing program.\nMinimum 2 years of clinical nursing experience; outpatient or ambulatory care experience preferred.\nExperience with patient care coordination, patient flow, scheduling, or clinical operations preferred.\nStrong knowledge of patient care processes, clinical documentation, HIPAA, and confidentiality requirements.\nProficiency with electronic health record (EHR) systems and standard computer applications.\nExcellent organizational, communication, and interpersonal skills.\nAbility to prioritize multiple tasks, resolve routine issues, and appropriately escalate clinical or operational concerns.\nDemonstrated ability to work effectively with patients, families, physicians, nurses, and multidisciplinary healthcare teams.\nStrong commitment to patient-centered care, teamwork, and quality improvement.\nAbility to work effectively in a fast-paced healthcare environment and adapt to changing program needs.\nWhat We Offer\nCompetitive compensation based on experience and qualifications: When determining the compensation, several factors may be considered including, years of relevant experience, credentials, union contracts, education, and internal equity.\nComprehensive health, dental, and vision insurance\n401K, Retirement savings plan with employer contribution\nGenerous Paid Time Off (PTO) and paid holidays\nTuition Reimbursement\nOpportunities for professional growth, development, and continuing education\nEmployee wellness programs and resources\nInfluenza Vaccinations are a requirement for employment.
