Assesses competencies, need for additional training and education, and develops and delivers educational programs to address issues.\nEssential Functions:\nAssists in identifying learning and development needs of staff using various data collection methods: questionnaires, chart review, focus groups. Based on findings, recommends education programs.\nProvides orientation to clinical staff and monitors effectiveness through competency tasks.\nAssists clinical staff with transition from new hire to job assignment using problem resolution and guided learning.\nProvides feedback to Clinical Leadership on new hire clinical staff based on evaluation of documentation and management of current caseload.\nEvaluates clinical staff performance in the field using field visit tool.\nAssists in the development, implementation, and updating of clinical policies and procedures to improve the quality of nursing practice and care management.\nDevelop and updates modules for clinical staff.\nConducts, develops and tracks Educational In-services for ElderServe Health Staff.\nParticipates in the Quality Improvement Programs; collaborates in setting standards of care and monitors effectiveness.\nRemains current on new developments by reading professional journals, attending professional seminars, and participating in professional associations etc.\nPerform any other job related duties as requested.\n Education and Experience:\nBachelor's degree required\nMaster's degree preferred\nEquivalent years of relevant work experience may be accepted in lieu of required education\nThree (3) years experience working in an acute care setting required\nStaff Development or adult instruction experience preferred\nprevious experience working in home care setting preferred\nCompetencies, Knowledge and Skills:\nEfficient with MS Office products\nAbility to translate staff needs into a variety of learning and performance improvement strategies\nExcellent written and verbal communications skills\nKnowledge of Department of Health and MLTC standards for home care\nKnowledge of Uniform Assessment New York (UAS-NY)\nKnowledge of Medicare and Medicaid regulations\nAbility to create, evaluate and present department, role-focused materials across topics with excellent public speaking skills\nAbility to develop, prioritize and accomplish goals\nListening and critical thinking skills\nStrong problem-solving skills with attention to detail\nStrong interpersonal skills and high level of professionalism\nTime management skills\nDecision making/problem solving skills\nStrong organizational skills\nChange resiliency\nLicensure and Certification:\nCurrent unrestricted Registered Nurse (RN) required\nWorking Conditions:\nGeneral office environment; may be required to sit or stand for extended periods of time\nAbility to travel as required by the needs of the department.\n$72,200.00 - $115,500.00 CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
