Summary:\nThe Maternity Care Coordinator is the member concierge and is the single point of contact for assessment, person centered planning, and service coordination for the Mom and Baby Beginnings program\nEssential Functions:\nTelephonically engage and enroll members in Mom and Baby Beginnings program\nEngage the member and complete a health and psychosocial survey, taking into account the cultural and linguistic needs of each member\nEducate members on the Mom & Baby Beginnings program\nConduct initial screening and needs assessments\nVerify eligibility, previous enrollment history, demographics, and current health status of each member\nProvide timely and accurate documentation\nBuild and strengthen member relationships by providing quality customer service\nEstablish positive, consultative relationships with assigned Physician offices\nDocument care coordination activities and member response in a timely manner according to standards of practice and CareSource policies regarding professional documentation\nLook for ways to improve the process to make the members experience with CareSource easier and shares with leadership to make it a standard, repeatable process\nWork closely with a members’ assigned case manager to coordinate care and programs\nUnderstand and implement Person-Centered Thinking\nFacilitate the Person-Centered planning process\nParticipate in person-centered care training in order to maximize the development of the Person-Centered plan\nCoordinate services and health benefits for members who meet criteria\nConsult and collaborate with other professionals and community members to coordinate care and develop Person-Centered plans\nOngoing communication with the internal complex clinical team\nMonitor service delivery to ensure appropriateness of care and compliance with waiver\nAssist Providers with technical questions regarding process\nMonitor and evaluate Person-Centered care plan on an ongoing basis through member, family, provider and stakeholder contact by modifying the plan as needed based on member choice\nAssist with internal care coordination activities to support member outcomes\nMaintain current and accurate documentation of contacts, treatment plans, case notes, referrals, and assessments in the electronic record according to current accreditation and compliance guidelines\nParticipate in meetings with providers to inform them of services and benefits available to members\nMay precept and/or mentor new staff\nPerform any other job duties as requested\nEducation and Experience:\nDiploma or Associates in a healthcare related field or equivalent years of relevant work experience is required\nBachelor’s degree in a healthcare related field is preferred\nA minimum of three (3) years of experience in a healthcare field (i.e. discharge planning, case management, care coordination, and/or home/community health management experience) is required\nCompetencies, Knowledge and Skills:\nIntermediate proficiency level with Microsoft Office, including Outlook, Word and Excel\nAbility to communicate effectively with a diverse group of individuals\nAbility to multi-task and work independently within a team environment\nKnowledge of local, state & federal healthcare laws and regulations & all company policies regarding case management practices\nKnowledge of and adherence to Case Management Society of America (CMSA) standards for case management practice\nStrong advocate for members at all levels of care\nStrong understanding and respect of all cultures and demographic diversity\nAbility to interpret and implement current research findings\nAwareness of community & state support resources\nCritical listening and thinking skills\nDecision making and problem-solving skills\nStrong organizational and time management skills\nTrauma informed training\nLicensure and Certification:\nNone\nWorking Conditions:\nGeneral office environment; may be required to sit or stand for extended periods of time\n$47,400.00 - $76,000.00 CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
