Founded in 1982 as a non-profit organization by members of the recovery community in Mesa, Arizona, Community Bridges Inc. has held as its principal mission “maintaining the dignity of human life” for more than 40 years.\n\nJob Summary\nThe Continuing Care Peer is an individual with lived experience in recovery and who provides community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations. This position provides peer support, outreach, care planning, and resource linkage both onsite and in the community. This Continuing Care Peer supports individuals during deflection encounters, sobering services, and transitions to appropriate levels of care, while ensuring follow-up, advocacy, and continuity of services\n\nSkills/Requirements: \nHigh school diploma or GED required.\nMinimum six months of recovery from substance use and/or mental health challenges.\nExperience in outreach, peer support, homeless services, or behavioral health preferred.\nDriver’s license (valid and in good standing) required.\nClear 39-month motor vehicle record required.\nPeer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.\n\nCBI is growing and expanding our services!\nWe are experiencing tremendous growth currently.
