Come talk to us about joining the Centerstone team!\nJOB DESCRIPTION:\nSUMMARY OF POSITION\nResponsible for providing wrap-around and/or intensive care coordination to youth and families with high needs. This includes in-person, community based child and family team meetings, IMCANS review, crisis services, prevention and safety plan review. Works collaboratively with other service teams to meet the needs of clients assigned via Pathways. May be responsible for assisting with linking clients to appropriate services, advocating on their behalf, providing support and monitoring effectiveness of services as needed.\nESSENTIAL DUTIES\nProvides Care Coordination and support for High Fidelity Wraparound (for Tier 1 clients) or Intensive Care Coordination (for Tier 2 clients). Works in consultation/collaboration with clients, family members and other providers to implement plan.\nProvides assertive outreach, advocacy, support and follow-up to assigned clients. Documents all outreach attempts and ensures all services/ outreaches indicated in the personal growth plan are provided in accordance with identified frequency.\nProvides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy. Provides case management to Family Support Program (FSP) and/or Specialized Family Support Program (SFSP) youth.\nAssists client in building a natural support team. Integrates natural community supports/resources, for any and all life domain area(s) identified as needing improvement, into the personal growth planning process and service delivery process. \nPresents relevant information for the purpose of assuring quality of care, gaining feedback, and planning changes in provision of personal growth-based services. Identifies and reports all Critical Incidents according to Centerstone policy. \nProvides community education as assigned.\nKnowledge, Skills, and Abilities\nEffectively communicate via written, verbal, in person and virtual methods.\nSkill in customer service/experience techniques.\nKnowledge of role and function of various community resources.\nSkill in problem-solving.\nKnowledge of basic service recipient’s dynamics of assigned disability groups.\nQUALIFICATIONS\nEducation Level\nBachelor’s Degree in a human services field, Associate’s Degree in human services with three years of field work in mental health, or High School education with five years of supervised field work in behavioral health may also qualify with approval.\nYears of Experience\nPrior experience required as detailed above.\nCertification/Licensure\nN/A\nTime Type:\nFull timePay Range:\n$16.50--$23.10Delivering care that changes people's lives starts with our employees.
