You’ll coordinate services and care for patients - providing coaching, strategies and coordination of health and wellness resources so they can live their lives more fully. This is a great opportunity if you’ve just graduated or if you’ve had a little experience but you’re ready for more.\nESSENTIAL DUTIES & RESPONSIBILITIES\nHelp patients navigate the health care system through coaching, education and support\nCoordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness\nRefers, advocates, and links client and family for additional services as identified.\nUse behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills\nProvides supportive and educational case management services to develop daily living and coping skills.\nResponsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.\nEnters written documentation into Electronic Medical Record in a timely manner for all services rendered.\nConnect with clients face to face in the office and community, telehealth, and by phone to address needs\nDemonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.\nAttend weekly staffing and team meetings and completes assessments\nAssist clients with developing natural support systems.\nCompletes all required agency trainings to maintain 100% compliance.\nMaintains departmental productivity expectations.\nAttend and participate in clinical supervision.\nKNOWLEDGE, SKILLS & ABILITIES\nQUALIFICATIONS\nEducation Level\nBachelor’s Degree in psychology, social work or related field required.\nYears of Experience\nExperience with Electronic Health Record preferred.\nCertification/Licensure\nValid driver’s license in good standing required.\nPHYSICAL REQUIREMENTS\nStanding - 10%\nSitting - 90%\nSquatting - Occasional\nKneeling - Occasional\nBending - Occasional\nDriving - Occasional\nLifting - Occasional\nDISCLAIMER\nThe duties and responsibilities described in this document are not a comprehensive list and additional tasks may be assigned to the employee from time to time, or the scope of the job may change as necessitated by business demands.\nTime Type:\nFull timePay Range:\n$20.05--$28.10Delivering care that changes people's lives starts with our employees.
