Come talk to us about joining the Centerstone team!\nJOB DESCRIPTION:\nSUMMARY OF POSITION - Intern\nMasters Level intern (social work/counseling), will work collaboratively with the discharge planner and CSU/WIC teams to develop comprehensive discharge plans for Clients admitted into the CSU.\nCommunicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.\nESSENTIAL DUTIES & RESPONSIBILITIES\nMeets one-on-one with each Client in CSU/WIC to collaboratively develop a discharge plan that addresses that Client’s presenting problem/primary needs:\nMental health: Outpatient appointment scheduling (psychiatric/counseling), safety planning, restricting access to lethal means, connection to follow-up programs\nPhysical health: PCP resources and appointments as needed\nSubstance Use: Relapse prevention plan, treatment resources and referrals as needed\nHousing: Resources and referral assistance as needed\nFinancial, Legal, Social: Resources and referrals as needed\nKNOWLEDGE, SKILLS & ABILITIES\nEffectively communicate via written, verbal, in person and virtual methods.\nPerform basic electronic medical record computer skills\nDetail oriented with effective problem solving skills\nKnowledge base in community resources\nSkill in crisis intervention\nAbility to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary.\nQUALIFICATIONS\nEducation Level\nBachelor's degree required.
