Employees are 100% vested after 4 years of credited service.\nReferral Bonuses: Earn up to $1,000 for referring new employees.\nWhat You’ll Do:\nDay School Referral, Intake, and Placement Coordination\nCoordinate the referral, screening, intake, admission, and placement processes for Therapeutic Day School students.\nServe as the primary point of contact for referring school districts and families, providing timely communication regarding referral status, requirements, and next steps.\nGather and review required referral, educational, and supporting documentation to facilitate screening and admission decisions.\nCoordinate and facilitate student observations, program tours, intake meetings, and other pre-admission activities as appropriate.\nCollaborate with appropriate CHS educational and clinical leadership to review referrals and support admission and placement decisions.\nCoordinate information, communication, and timelines to support timely placement decisions and a smooth, successful student entry into programming.\nStudent Transition and Reintegration\nCoordinate transition planning and implementation for students exiting Children’s Home Society School programs\nCollaborate with students, families, CHS educational teams, referring districts, and receiving schools throughout the transition process.\nDevelop individualized transition plans addressing each student’s educational, behavioral, social-emotional, and support needs.\nCoordinate communication, information-sharing, transition meetings, school visits, and other activities to promote continuity of services and supports.\nIdentify potential barriers to successful reintegration and collaborate with teams to develop proactive strategies that support transition to less restrictive educational settings.\nProvide follow-up consultation and support to students, families, and receiving schools to promote adjustment, problem solve concerns, and strengthen transition outcomes.\nSchool District Partnerships\nDevelop and maintain collaborative relationships with public school districts and other educational partners.\nServe as a liaison between CHS therapeutic educational programs and partner schools, promoting effective and consistent communication\nBuild awareness and understanding of CHS therapeutic educational programs, services, and referral pathways.\nIdentify emerging needs, trends, and challenges experienced by partner schools in supporting students with complex educational, behavioral, and social-emotional needs.\nCollaborate with educational partners to problem-solve barriers, strengthen partnerships, and identify opportunities to better support student success.\nTherapeutic Education Outreach & Training\nCollaborate with appropriate CHS educational and clinical leadership to develop and provide training and educational resources for public schools and community educational partners.\nIdentify training needs and priorities in partnership with schools and districts and adapt content to their specific needs.\nProvide training related to trauma-responsive and relationship-based practices, emotional regulation, behavioral support, classroom strategies, and sensory and environmental supports.\nTranslate CHS expertise in therapeutic education into practical, school-based strategies and resources that can be implemented within partner schools.\nCoordinate training logistics, materials, communication, and follow-up resources to support effective delivery and implementation.\nGather participant and partner feedback to inform future training opportunities and continuously improve outreach efforts.\nEducational Consultation\nCollaborate with appropriate CHS educational leadership to provide consultation and practical support to partner schools serving students with significant behavioral, emotional, and educational needs.\nAssist school teams in identifying barriers to student success and developing practical strategies and supports within the student’s local educational environment.\nHelp teams translate therapeutic, behavioral, and relationship-based concepts into sustainable school-based practices.\nCollaborate with school teams to identify and strengthen classroom, environmental, behavioral, and student-specific supports that promote successful participation in school.\nProvide follow-up consultation and collaborative problem-solving as schools implement strategies and respond to changing student needs.\nSupport partner schools in building local capacity and sustainable practices that strengthen their ability to successfully serve students with complex needs.\nProgram Development, Data, & Quality Improvement\nSupport development, implementation, and continuous improvement of Therapeutic Day School and educational partnership initiatives.\nTrack and maintain data related to referrals, intakes, placements, transitions, outreach, training, consultation, and partnership activities.\nReview data to identify trends, emerging needs, service gaps, and opportunities for program improvement or growth.\nGather and incorporate feedback from school districts, families, educational partners, and CHS teams to inform program development.\nCollaborate with CHS leadership to evaluate program effectiveness, partnership outcomes, and progress toward established goals.\nContribute to quality improvement initiatives, reporting, and development of processes and resources that strengthen educational programming and partnerships.\nTeam Collaboration & Professional Responsibilities\nCollaborate with CHS educational, clinical, residential, administrative, and leadership teams to support coordinated programming and effective educational partnerships.\nMaintain effective communication and information-sharing across CHS teams to support students, families, and partner schools.\nParticipate in team meetings, committees, program development, and quality improvement activities as assigned.\nMaintain required training, professional competencies, documentation, and organizational expectations associated with the position.\nRepresent CHS professionally and collaboratively in interactions with students, families, school districts, community partners, and other stakeholders.\nDemonstrate and promote CHS values, of our employees and clients.\nRequirements\nEducation:\nRequired\nAssociate degree in education, human services, behavioral sciences, social services, or a related field, or an equivalent combination of relevant education and experience.\nPreferred\nBachelor’s degree in education, special education, human services, behavioral sciences, or a related field. Additional training or education in special education, therapeutic education, trauma-responsive practices, behavioral support, or related areas.\nExperience:\nRequired\nExperience working with children and adolescents with educational, behavioral, and/or social-emotional needs. Demonstrated ability to build collaborative relationships and communicate effectively with families, professionals, and community partners.\nPreferred\nExperience in school-based or therapeutic educational settings, including collaboration with school districts.
