As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions-reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.\nLocation(s):\nNew York City, Staten Island, Yonkers\nWhat You’ll Do\nDirect patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.\nTech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.\nData into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.\nClose the loop: Complete documentation/charts within the same day\n High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.\nQuality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting\nMentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.\n Regional flexibility: Cover sites within a 30-40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.\nHow Success is Measured (KPIs)\nVisits: Target 30 patient visits/day\nEngagement: ≥80% Time to Acknowledgement Rate: Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.\nAdoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.\nMust-Have Qualifications\nCertified and licensed Nurse Practitioner in NY; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.\nEligible for Medicare participation;
