Assists the treatment team in formulating discharge plans and service planning for all clients in inpatient settings. The Discharge Planner will participate in hospital staffing and treatment team meetings to assist in inpatient discharge planning. Assists outpatient team in any Court Ordered Paperwork as required for admissions and discharges, including amendments. Works with the Court Coordinator to ensure paperwork is completed accurately and timely.\nWhat you’ll provide:\nBachelor's Degree in behavioral field or any combination education/experience that meets BHT requirements as stipulated by AZDHS/BHS required. \nMaster's Degree in the behavioral health field is preferred.\nMinimum of 2 years of case management or inpatient experience in behavioral health is required.\nDetail oriented and ability to multi-task in a fast-paced environment.\nMust have Fingerprint clearance card or be eligible to obtain one.\nMust have a valid AZ driver's license with no major infractions in the past three years.\nMust have regular access to reliable transportation.\nBi-lingual (Spanish) Diff per hour available.\nMust be 21 years of age or older at time of employment. \nAdditional Requirements and Responsibilities:\nDemonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.\nMaintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency.\nMaintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.\nIf driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required.
