This role guides and develops Support Coordinators, promotes high-quality, person-centered services, and ensures program operations meet applicable regulatory, contractual, and documentation requirements.\nThis is a great opportunity for someone who enjoys leading and mentoring a team, strengthening service delivery, and making a meaningful difference in the lives of individuals and families. You’ll help Support Coordinators navigate complex needs, build their professional skills, and connect individuals with community-based resources while ensuring program quality and compliance.\nKey Responsibilities\n\nLead, train, supervise, and evaluate Support Coordinators while supporting effective time management and professional development.\nOversee targeted case management services across three counties, ensuring quality, consistency, and compliance with state regulations and contractual requirements.\nReview and approve Individual Support Plans, assessments, budgets, amendments, and supporting documentation.\nProvide guidance to staff on case management practices, administrative rules, regulations, and community resources.\nCoordinate case assignments, transitions, and transfers of individuals between counties and service areas.\nCollaborate with regional partners and attend meetings to stay informed of program updates, requirements, and changes.\nPrepare and maintain documentation for audits and reviews and support ongoing program quality.\nParticipate in Individual Support Plan meetings and provide direct coverage for Support Coordinators when needed.\nTravel between locations to provide program oversight, staff support, and operational coverage as needed.\nMaintain a safe, professional work environment and apply approved crisis intervention techniques in accordance with certification and organizational protocols.\n\nRequirements, Skills, Knowledge and Expertise\nBachelor’s degree in social work, psychology, counseling or related field required\nA minimum of Two (2) years of relevant experience required\nA valid, unrestricted driver’s license and agency established minimum automobile coverage required\nQUALIFICATION(S): LICENSURE/CERTIFICATION\nMust obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hire\nQUALIFICATION(S): ADDITIONAL
