This role ensures the smooth functioning of the center, supports staff, and provides excellent service to students and families.\nKey Responsibilities\nCoordinate daily operations of the SBHC.\nMaintain confidentiality regarding patients, staff, and program matters.\nAssist in scheduling and maintaining clinical schedules.\nLocate and escort clients throughout the school buildings.\nVerifies insurance coverage as indicated by CS-HHC billing procedures.\nReceives telephone calls from patients and correctly and accurately records and transmits messages for Providers. Initiates outbound calls as needed.\nCoordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary\nPrepares patients and rooms for exams according to specific procedures.\nAccurately and legibly document vital signs, including blood pressure, pulse, respiration, temperatures, O2, saturations and finger sticks.\nPerforms hearing and vision tests, Urine Dipsticks, Urine Pregnancy Tests and other CLIA waived tests when necessary.\nMaintains accurate logs of point of care testing, controls, and expiration dates.\nEnsures that the necessary equipment and information are available for exams.\nMaintains regulatory readiness and completes monthly EOC documentation.\nProvide support for classroom presentations and parent presentations.\nParticipates in Outreach and Marketing development and activities to promote School Based Health and CS-HHC.\nHelps patients and their families understand how to utilize the resources of the CS-HHC.\nServes as a backup site coordinator for the CT VFC Program and maintains all appropriate temperature logs.\nMaintains patient enrollment information for the site and ensures accuracy.\nManages patient records using EPIC and any other technologies as required per funding source (i.e. HealthX/BrightCare)\nParticipate in a minimum of 4 outreach events annually, these may include hours outside the regular school day (orientation, parent/teacher conference night, welcome back to school night) \nParticipates and attends school events (i.e. assemblies, social emotional learning days, field day, school performances)\nWorks with teachers, administration, and support staff to identify and enroll students in SBHC.\nRegular and on-going communication with school nurses, staff, and administration.\nAttends weekly school collaboration meetings to consult and obtain referrals. (i.e. SSST, Attendance, SPMT)\nSchool Based Coordinator II\nMobile Van Driver:\nDrive the mobile van to various locations as needed.\nMaintain mileage, travel logs, and prepare reports as needed.\nMaintain fuel levels and adhere to preventative maintenance schedule.\nMaintain a clean and mechanically-sound vehicle and ensure clinic is fully operational\nAssist in coordination of basic vehicle maintenance and repairs.\nAssist with delivery of patient care services and management, and navigation of participants on and off the mobile unit\nPerforms equipment checks, pre-tripand post-trip inspections, and ensures overall cleanliness before, during, and after visits\nMaintains supply inventory\nOther duties as required to support the Mobile Medical Vehicle’s readiness to operate and to accept participant encounters\n Required Qualifications\nHigh School Diploma/GED required.\nCPR certification required.\nMust have diploma from accredited Medical Assistant or Certified Nursing Assistant program.\nBilingual English/Spanish strongly preferred.\nOne (1) years’ experience as a Medical Assistant or Certified Nursing Assistant preferred. Previous medical office or school administration experience is preferred.\nMobile Van Drivers: Tractor/trailer/bus driving experience preferred or graduated from an acceptable truck driving training program and have 2 years of driving experience.
