As a Nutritional Support Coordinator, you’ll play a vital role in helping patients with chronic conditions achieve better health through nutritional education, reinforcement, and compassionate care coordination.\nWhat We’re Looking For:\nWe’re looking for a compassionate, patient-centered Nutritional Support Coordinator to support patients’ nutritional care through remote education, reinforcement, and care coordination. The ideal candidate has a healthcare background-whether as a health coach, care coordinator, medical assistant, or LPN-with experience or strong interest in nutritional support for chronic conditions. You excel at simplifying health concepts for diverse populations, know when to escalate concerns to clinical staff, and are passionate about helping patients follow through on their care plans. You thrive in fast-paced environments, build rapport easily with patients, and collaborate effectively with clinical teams.\nWhat You’ll Do:\nPatient Outreach & Nutritional Counseling \nManage an assigned panel of 300-350 patients and conduct required monthly nutritional outreach and support.\nConduct 50-60+ daily phone calls with patients, providing nutritional care plan support and health coaching tailored to their specific conditions.\nMaintain call quality standards and professional communication in every patient interaction; pass regular QA call reviews.\nNutrition Planning & Patient EducationEducate patients on nutrition concepts, dietary modifications, and weight management strategies. \nSimplify complex nutritional information for diverse patient populations.\nReinforce provider-directed nutritional guidance and support patient adherence to established care plans. \nSimplify complex nutritional information for diverse patient populations.\nCare Coordination & Collaboration Collaborate with clinical teams, providers, and caregivers to identify and address nutritional and clinical needs. \nSupport care coordination goals by staying on schedule with monthly outreach targets. \nEscalate patient concerns or changes in condition to appropriate clinical staff per established protocols.\nDocumentation & Compliance Document all patient interactions in real time using the care coordination platform. \nMaintain HIPAA compliance in all patient communications and documentation. \nEnsure accurate documentation to support care coordination goals.\nRequired Qualifications:\nA Degree in Dietetics or Nutrition.\n2+ years of experience in clinical nutrition counseling or dietary care coordination.\nExperience supporting patients with chronic conditions in in-home, in-facility, or remote settings.\nMinimum 2+ years of clinical nutrition counseling or dietary care coordination experience.\n2+ years of Electronic Medical Records (EMR) and computer navigation experience.\nDemonstrated ability to educate patients and simplify complex nutritional concepts.\nHIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.\nMust be legally authorized to work in the United States.\nPreferred Qualifications:\n3+ years of experience in healthcare nutrition, chronic care management, or patient education.\nCertification as a:Registered Dietetic Technician (NDTR), or\nCertified Nutritionist, or\nCertified Health Coach\n\nBilingual (Spanish/English) preferred.\nExperience in remote patient care or telehealth nutrition counseling.\nExperience with care management or chronic disease management programs.\nFamiliarity with weight management or behavioral health principles.\nPrevious experience managing large patient panels (200+).\nCompensation & Benefits:\nCompetitive hourly rate.\nPaid Time Off + Company Holidays.\nMedical, Dental, Vision Insurance.\nComplimentary Life Insurance.\n401(k) Plan.\nOptional Short-Term, Long-Term Disability, Critical Illness & Accident coverage.\nEmployee Assistance Program including mental health resources.\nCompany-provided equipment (laptop, monitor, headset, etc.).\nWork Environment & Requirements:\n100% WFH with a Monday-Friday schedule-no weekends or late nights.\nCompany-provided equipment (Mini PC, monitor, headset, etc.)\nHigh-speed broadband internet and private home workspace requiredCandidates must complete a company-provided internet speed test to confirm a minimum of 50 Mbps download / 5 Mbps upload.\n\nPrivate workspace for compliance with HIPAA privacy laws.\nIf you’re a compassionate healthcare professional ready to make a meaningful impact in patients’ lives, we want to hear from you.
