Come grow your career and make a meaningful impact as an MDS Nurse at Creekside Health and Rehabilitation in Indianapolis, Indiana.\nResponsibilities\nMDS Coordinator Primary Responsibilities\nThe MDS Coordinator is responsible for leading the MDS process for the facility per the RAI Guidelines.
