The manager will participate in referral management, process improvement, and change management.\nDirect/Indirect Patient Care:\nProvide direct patient care to patients enrolled in our palliative care program by phone or video visit during scheduled and on-demand patient visits.\nParticipate in and/or lead patient panel reviews\nParticipate in weekly IDT (1h/wk)\nPeople Management:\nClinical champion for the work - you are a player-coach, modeling clinical excellence for other palliative care clinicians.\nModel and support development and implementation of new workflows during rapid cycles of iteration\nDirect supervisor of palliative care clinical staff (mix of supervisees TBD - NP/RN/SW)\nGoal setting and coaching of direct reports to achieve business goals as set by leadership\nRegular review of direct reports’ work and holding RNs and Social Workers accountable to individual performance. Creating performance improvement plans, when needed.\nPerform regular clinical audits (QA) that include a mix of note review and listening tours\nc. Steward our member-centric and open clinical culture to our members and\ncolleagues\nRecruitment, interviewing, hiring, and onboarding of new clinical staff\nAttributes to success:\nService-oriented clinician and leader - you participate and help develop engaged teams that have a deep culture of service to members/patients\nHighly organized and detail-driven\nWorks well in a fast-paced environment with firm deliverables and timeframes\nCuriosity to recognize process deficiencies, recommend improvements and partner to implement solutions\nExcited to be an active participant as a leader and do what it takes to achieve fast-paced forward progress\nPreference toward collaboration and preventing silos; be a team-player and able to work in a cross matrix setting\nSkills and experience:\n5+ years of experience as a Palliative Care Clinician\n2+ years of experience leading front line clinical teams with accountability for team level outcomes\nDemonstrated ability to champion new initiatives and achieve direct business outcomes\nYou have great clinical and non-clinical judgment and are able to coach around performance to drive clinical effectiveness\nYou are deeply empathetic and humanistic, and want to go the last mile for our patients\nYou work well with individuals from varied backgrounds and skill-sets\nYou are committed to this mission: you know the road will be bumpy, stressful, and require compromises, but you’re up for the ride\nYou learn from every experience and are not afraid to fail - that's how you're wired\nYou’re a great team member with a can-do attitude\nSalary range: $140,000 - $170,000\nThe pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting.
