These can include topics such as wellness, coping skills and mindfulness. Services can also assist students to develop tools to manage challenges at school, home, work, and in the community. Mercer County College Connects offers these supportive services through screening, assessment, community linkage to physical and behavioral health treatment.\nSchedule: Full-time; 40 hours; Monday- Friday\nPay: $55,000\nCompetencies:\nAbility to provide Person Centered, Family-Centered, and Trauma-Informed Care; Understanding and commitment to supporting recovery and resiliency; Ability to assess and interpret information related to population; Knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; General understanding of treatment needs; Knowledge of available treatment resources and appropriate use; Population-specific communication skills; Understanding of Care Coordination; General knowledge of housing subsidies and management of the subsidies.\nResponsibilities:\nPromote consumer-driven services and recovery-oriented environment;\nAssist individuals and their family in making decisions, taking action, and treatment planning.\nProvides education and information to individual and family\nProvide screening, assessment, linkage and brief counseling to individuals seeking services\nInterfaces with the internal and external providers to coordinate linkage and warm-handoffs to services\nProvides in-person and telephone follow-up with all individuals to ensure adequate linkage and stabilization with services\nDocuments all interactions with individual, maintains accurate and up-to date information in the individual's electronic health record both for documentation and billing purposes\nAides with on-campus marketing and program development\nCoordinates with partners at Mercer County College\nAttends in-person supervision and team meetings both off-site and on-site\nDevelops recovery plan and supports the individual\nResponsible for maintaining expected level of service and productivity as defined by program management\nPerforms other related duties as required\nBenefits:\nCompetitive base salary\nMedical and dental insurance\nVision plan\nRetirement plan\nFlexible spending plans\nEXCELLENT time benefits for qualified positions!\nOpportunity for personal and career growth, including supervision towards professional licensure for eligible candidates\nTeam-oriented environment - we practice the FISH!
