To provide unbiased and person-centered education, advocacy, assistance and support to NH residents so they may retain their dignity, quality of life and highest level of independence. Support individuals in a manner which would allow them to make informed choices/decisions regarding their long-term service and support and assist individuals with navigating the various community organizations, agencies, and other resources in the community.\nSchedule: Full time 40 hours a week; Monday-Friday 8a-4:30p or 8:30a-5p\nLocation: Must be willing to work between the Manchester and Nashua offices\nCompensation: $21/hour\nEssential Responsibilities: \nPerforms an assessment of the contact/caller’s situation/needs and provides the contact/caller with assistance in referrals to long-term supports and services through telephone contacts, walks-ins, emails, home visits/community settings and multi-media.\nProvides information on and referral to community resources which may meet the needs of the contact/client.\nProvides long term support counseling to select individuals in hospitals, rehab facilities, and/or nursing homes.\nCompletes eligibility screening and provides information to assist individuals in understanding and applying for State and Federal program.\nCompletes documentation on all contacts using assigned database following all protocols/processes as mandated by the contract and in compliance with InformUSA.\nComplete follow-ups on contacts following InformUSA protocols and contract requirements\nComply with completion of reporting requirements mandated by the ADRC contract.\nProvide Medicare information and counseling to Medicare beneficiaries especially during the Medicare Annual Enrollment Period.\nAttends meetings as mandated by contract or required by State.\nAssists with general office support including by not limited to answering phones, assisting, walk-ins and other office tasks as needed\nObtain all mandatory certification as required by the contract which includes the InformUSA (Alliance for Information and Referral Certification (AIRS)); State Health Insurance Assistance Program (SHIP) Counselor within six (6) months to one (1) year of hire.\nQualifications:\nBachelor’s Degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR\nFive (5) years’ experience working directly with programs supporting the elderly or disabled population in lieu of degree Basic knowledge of community long-term care support services for the elderly and disabled population, OR\nCombination of education and experience totaling five (5) years\nBasic knowledge of community services such as home health agencies, assisted livings, nursing homes, and hospitals\nBasic knowledge of State and Federal program such as Medicaid and Medicare\nStrong oral, written and interpersonal skills\nAnalytical and problem-solving ability\nComfortable with performing home visits and community outreaches\nBasic level of computer skills to include Microsoft Office application suite and database programs\nValid driver’s license, auto insurance, and reliable transportation\nWhat’s in it for you?
