Your Role  

As the Implementation Manager for Evergreen Connected Care (ECC), you own the day-to-day execution of practice onboarding and the ongoing operating relationships with our partner practices. Connected Care is delivered as a white-labeled extension of the practice and its providers; you are the person who makes each launch happen on time and keeps the relationship running well afterward. 

You run the standard onboarding sequence at each practice - kickoff, implementation and breakout meetings, final all-hands, and the controlled outreach start - and you hold the timeline. You build and refine the meeting materials, facilitate sessions alongside the Program and Clinical Director or on your own, track every open item to closure, and make sure the right people know what is expected of them and by when. 

This is an execution and coordination role. The operating model already exists; your job is to run it consistently as the program scales and keep work from falling through the cracks. This position is fully remote, with the possibility for occasional travel to on-site meetings with practices. 

Role Responsibilities  

Some responsibilities may vary based on the practices you support, but this role's primary duties include the following: 

• Practice Onboarding and Implementation  
  • Run the ECC onboarding sequence end-to-end for each new practice, from kickoff through first outreach and the post-launch ramp review. 
  • Hold the working timeline across multiple concurrent implementations and escalate slippage early. 
  • Coordinate the parallel implementation and clinical workstreams so the two tracks stay in sync, including patient data intake and validation, forms and materials, outreach wording, after-hours handling, billing pathway, and system access. 
  • Confirm and maintain the practice contact map: leadership, project champion, clinical lead, data/EHR, and billing. 
• Meetings and Materials  
  • Build agendas and prepare, refine, and version the meeting materials and decks used across kickoff, breakout, and all-hands sessions. 
  • Facilitate meetings with the Program Director or independently and drive each session to decisions and named owners. 
  • Capture and circulate follow-ups after every meeting, and confirm the practice completes its post-meeting forms and deliverables. 
  • Prepare recurring internal and practice-facing status updates and materials for leadership review. 
• Partner Practice Relationships  
  • Serve as a primary day-to-day point of contact for partner practice leadership, administration, clinical leads, billing, and EHR/data contacts. 
  • Manage expectations on timing, data requirements, and practice-side responsibilities, and keep communication timely and clear. 
  • Surface practice-side risks and blockers to the Program Director before they affect launch dates or patient outreach. 
• Tracking, Follow-Through and Process  
  • Maintain a single source of truth for deadlines, commitments, and open items across all practices, and drive them to closure rather than simply reporting them. 
  • Partner with the Clinical Program Director on coordinator staffing readiness, training timing, and launch pacing. 
  • Coordinate with internal Finance, Legal and Compliance, Data, People Operations, and Market Operations counterparts to unblock practice-facing work. 
  • Manage vendor and platform coordination required for launch and steady state, including the technology partner's implementation and account teams. 
  • Identify where the process breaks repeatedly and propose durable fixes - checklists, templates, trackers - so each launch is cleaner than the last. 
• Other duties consistent with this role, as assigned. 

Required Qualifications  

• 5+ years of experience in implementation, program or project management, client success, or practice/provider operations, ideally in healthcare services, value-based care, or healthcare technology. 

• Demonstrated experience running multi-stakeholder implementations to a defined timeline with external customers or partners. 

• Excellent written and verbal communication skills, including the ability to build a clean deck and facilitate meetings of clinicians and executives. 

• Strong organizational discipline and follow-through; comfortable operating with autonomy and ambiguity. 

• Advanced skills with MS Office Suite of products including Outlook, Teams, SharePoint, Excel, and PowerPoint. 

• Willingness to travel occasionally for practice kickoffs and on-site sessions. 

• Able to work effectively in a primarily remote environment: 
  • Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended 
  • Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role 
  • Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information 

Preferred Qualifications  

• Experience with nephrology, chronic care management, or other CMS care management programs. 

• Familiarity with provider practice operations, EHR data workflows, and secure patient data transfer. 

• Experience managing vendor or platform partner relationships. 

• Comfort learning care management platforms and reporting tools quickly. 

Compensation  

The pay range for this role is $100,000-$110,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.   

This role is also eligible for an annual bonus.
