The Health Information Coordinator works in conjunction\nwith the interdisciplinary team to ensure complete integrity of patient information.\nThe hourly pay range for the Health Information Coordinatorposition at Center for Elders’ Independence is $22.96to $34.43. Placement within the range is determined based on job-related factors such as relevant experience, skills, abilities, internal equity, and market data, consistent with CEI’s annual compensation review and established compensation practices\nDuties and Responsibilities:\nEnters data and makes changes in CEI’s electronic health record, including\ndocumentation of POLST.\nScans and files documents to electronic record in a timely manner.\nConducts audits of the records to maintain accuracy and completeness of the\nelectronic health record, consistent with federal and state regulations, HIPAA,\nand CEI policies and procedures.\nIdentifies and reports documentation deficiencies to team members and to\nleadership to ensure deficiencies are resolved.\nObtains outside medical records from external providers / other facilities for\ncurrent participants including new enrollees’ medical records and enrollment\ndocuments and informs all necessary parties once received; scans documents\ninto electronic record.\nMonitors clinic fax inbox for any clinical reports, renames reports and ensures\ntimely delivery to appropriate provider.\nResponds to requests for access to participant medical records from participant\nor family, and other outside sources, including obtaining appropriate approvals\nfor release prior to releasing the records.\nMaintains monthly deactivation lists/enrollment status in electronic record.\nCollaborates to prepare records for CMS or otherregulatory agency audit.\nMedical Records Tracking to include filing, storage and retention of active and\nclosed medical records and health information including recording submission of\nmedical records to off-site storage.\nEnsures adherence to HIPAA compliance standards at all timesto ensure\nconfidentiality of participant’s private information.\nPerforms other duties as assigned in a positive and helpful manner.\nMinimum Qualifications:\n2+ years’ experience in medical records position preferred.\nDemonstrated ability working in an electronic medical records environment.\nPrefer individuals with one of the following: Registered Health Information\nTechnician (RHIT), Registered Health Information Administrator (RHIA), or\nBachelor’s Degree in Health Information Management.\nKnowledge of HIPAA compliance standards.\nExcellent skills with Microsoft Office Suite.\nStrong organizational skills and attention to detail.\nStrong communication and interpersonal skills.\nDemonstrated ability to handle confidential information\nThe above job description is intended to communicate the general function of the\nmentioned position and by no means shall be considered an exhaustive or complete\noutline of the specific tasks and functions that will be required.
