The goal of the position is to support clinicians in the delivery of quality of patient care through innovative and cost-effective best practices. This position promotes multidisciplinary integration to ensure successful continuity of care.\nEssential Job Functions\nResponsible for coordinating hospital admissions and transfers\nFacilitates conference calls between providers to coordinate continuity of care\nIdentify, resolve, and escalate issues, process deviations, and service failures to leadership\nMaintain regulatory compliance with all state and federal regulatory agencies including, but not limited to, CMS Conditions of Participation, EMTALA, DNV, and HIPAA\nAdhere to all company and departmental values, policies, processes, and procedures\nMaintain detailed and accurate documentation of all admissions, transfers, conference calls, and all other activities related to essential job functions\nCommunicates accurately and timely with a multi-disciplinary team including but not limited to providers, nurses, administrators on-call, hospital leadership, partner facilities, and non-partner facilities\nEstablish good rapport and cooperative working relationships with partner transfer centers, non-partner transfer centers, and all other referral sources\nMaintain a positive, professional, and collaborative demeanor in all interactions (oral, telephone, written, and electronic)\nProvide superior customer service by listening, communicating empathetically and effectively in all interactions (oral, telephone, written, and electronic) \nAdapt to change with a willing and innovative spirit\nMeets all individual and departmental goals and quality standards as assigned\nSafeguard sensitive and confidential company information including patient PHI in remote work environment\nCollaborate with Transfer Center Coordinators for clinical expertise in gathering and reporting appropriate clinical data to ensure continuity of care \nPerform other duties as assigned\nBasic Qualifications\n2 years hospital related or Emergency Room experience, required\nHS Diploma, required\nHealthcare transfer center experience, preferred\nSuperior interpersonal skills\nMaintain a high degree of professionalism\nAbility to manage multiple tasks simultaneously\nAssume personal responsibility for actions \nPossess a strong technical aptitude\nDemonstrate a friendly and service-oriented demeanor\nStrong attention to detail, organizational skills and follow-through discipline required\nPosition requires fluency in
