As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.\nEmpath Health is currently seeking an RN PRN to join our team.\nCoverage area includes theSouth Sarasota and Charlotte county\nWhat you'll Do\nThe PRN RN Case Manager creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient’s hospice experience. When a patient and loved ones think of Tidewell Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.\n\nThe PRN RN Case Manager reviews and updates the care plan, physician orders, pharmacy profile and other records. The nurse also educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.\n\nThe successful applicant will serve hospice patients and their families in the home, long-term care facility, or assisted-living facility.\n\nApplicants must be a graduate of an approved nursing program and own a current Florida RN license. BSN or MSN preferred.
