Our goals are accomplished by the commitment of every employee.\n ETS is an advocate for our families and providers by practicing the following values and beliefs:\nResourceful- We are proactive and solution-based in all our actions within our team and community.\nEthical - We are upstanding professionals that provide services by upholding our highest principles, including communication, interactions, and standards of practice.\nAcceptance - We are empathetic and honest providers who support our families and each other.\nProfessional - We are responsible leaders that hold ourselves and each other accountable.\nService Coordinator Job Description\nEssential Duties:\nProcess referrals, schedule initial meetings with families, conduct screenings using standardized protocol and tools approved by AzEIP, and ensure timely delivery of services\nAssess nature of clients' situations by interviewing and reviewing personal history\nEnsure proper maintenance of child files, required documentation and the AZ DES automated system\nCoordinate, attend and schedule assessments; IFSP (6 months and annual), IEP, eligibility determination meetings, transition planning; notify all interested parties of meeting dates and times\nCommunicate client updates to all relevant parties\nFacilitate referrals to other healthcare professionals and programs\nDemonstrate knowledge in AzEIP Policies and Professionalism, Child Development, Family Professional Partnerships, Teaming, Assessments and Evaluation, Planning, and Intervention\nDemonstrate knowledge in AZ DDD/DES policies\nQualifications:\nA Bachelor's Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education, or a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of coursework and approval by AzDES/AzEIP.\nCandidate must successfully complete a background clearance and obtain Level One Fingerprint Card Clearance\nTravel to attend meetings, training and appointments\nExcellent written and verbal communication skills\nBilingual English/ Spanish for this position is a plus\nW2 Employee Benefits:Medical, Vision, Dental, Paid Holidays, 401k, Life Insurance, Long Term Disability, Pet Insurance, and more perks\n We are an agency of providers dedicated to empowering families and their loved ones by enhancing their quality of life within our local communities.
