The ETSU College of Nursing, Office of Practice invites applications for a full-time Care Coordinator (RN) for their Johnson City Community Health Center clinic.

The Care Coordinator (RN) partners with chronically ill or high-risk patients, their families, healthcare providers, and community resources to ensure timely access to care, increase preventative services, and reduce hospital readmissions and emergency visits. By developing personalized care plans and offering culturally appropriate education, the Care Coordinator empowers patients to manage their health more effectively and improves adherence to treatment.

This role involves daily one-on-one patient meetings, coordinating follow-ups with healthcare teams, and connecting patients to community resources. Success in this role leads to better patient outcomes, increased satisfaction, and reduced healthcare costs.

Knowledge, Skills, and Abilities

  • Evidence of essential leadership, communication, education, and counseling skills.
  • Proficiency in communication technologies (email, cell phone, etc.).
  • Highly organized with the ability to keep accurate notes and records.
  • Experience with health IT systems and reports is desirable.
  • Local knowledge about and connections to community health care and social welfare resources is desirable. The ability to speak a relevant second language is desirable.
  • Core values consistent with a patient- and family-centered approach to care.
  • Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal, and nonverbal.
  • Demonstrates a positive attitude and respectful, professional customer service.
  • Acknowledges patients’ rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations.
  • Proactively acts as a patient advocate, responding with empathy and respect to resolve patient and family concerns, and recognizing opportunities for improvement to meet patient concerns.
  • Proactively continues to educate self on providing quality care and improving professional skills.

While performing the duties of this job, the employee is occasionally required to position themself and balance. The employee must occasionally move (including lift, carry, push, and pull) up to 100 pounds or more. The employee must be able to perceive and respond to sounds and alarms; communicate effectively with others; be able to observe and monitor situations and details at close range in full and dimmed light. The employee may be required to move and/or stay in a stationary position in various settings for up to 7.5 hours at a time in addition to preparing and closing a clinical shift. Rapid mobility may also be required on occasion. Physical expectations of an assigned clinical facility may vary and be subject to change.

Stress levels may be high due to frequently dealing with numerous students, patients, family members, and other healthcare providers. This position demands concentration and the ability to work in emotionally charged situations.

Required Qualifications

  • Associate’s degree in Nursing.
  • Three years of relevant experience in clinical or community resource settings.
  • Active, unencumbered Registered Nurse licensure in the State of Tennessee or multi/compact state.
  • Experience with Chronic Care management, Transitional Care Management, and/or Patient-Centered Medical Home.

Preferred Qualifications

  • Bachelor’s degree in Nursing.

College of Nursing Overview

The College of Nursing is part of the University’s Academic Health Sciences Center, which also includes the James H. Quillen College of Medicine, the Bill Gatton College of Pharmacy, the College of Public Health, and the College of Health Sciences. The College of Nursing is the largest in the State of Tennessee offering multiple tracks for students enrolled in undergraduate and graduate programs. The Department of Undergraduate Programs is home to over 45 full-time faculty and 700 students across five (5) tracks including the Traditional BSN, Accelerated BSN (Johnson City, Kingsport, Sevierville), LPN-BSN, and RN-BSN/Dual Degree. The Department of Undergraduate Programs boasts a record of excellence with 60% of faculty holding specialty certifications and a strong record of collaboration and successful student outcomes. All programs in the College of Nursing are fully accredited by the CCNE.
