identify and link with needed services and supports to improve their quality of life within the home and the community.\nAs a Support Coordinator, you must be willing and able to travel and drive throughout the State of NJ as well as have a flexible schedule to accommodate the individuals schedule including evening and weekend hours.\nDUTIES AND RESPONSIBILITIES:\nDevelops individual plans of care, with input from the individual and family, which is based on the personal preferences and choices of the individuals being served.\n\nUtilizes available specialized and generic community resources in combination with other formal and informal supports and services.\n\nMaintains continuing contact with individuals and families as individual plans of care are put in place.\n\nAll Support Coordination deliverables as outlined in the Policy & Procedure Manuals (SP & CCP).\n\nAssist individuals and their families in coordinating, selecting, and overseeing their own supports.\n\nWorks as part of a regional support team.\n\nContinues learning about relevant, local accessible services and providers.\n\nInforms providers and the Division about identified service gaps.\n\nCompletes required training and attends meetings as assigned.\n\nMay perform other duties as assigned by the CEO, SVP of Programs and Services, Director, or SC Supervisor as need arises.\n\nQualifications\nQualifications/ Background:\nBachelor’s Degree in a relevant discipline and 1-year equivalent experience working with adults (18+) with intellectual or developmentally disabled population;\n\nComputer Proficient (MS Office Outlook, Word, Excel, PowerPoint);\n\nAble to solve problems independently, effectively and creatively;\n\nExcellent Organization and Communication skills;\n\nExcellent time management skills to be able to juggle multiple plans for multiple families simultaneously;\n\nBalance of work in the field and remote office- Must have a flexible schedule to meet the needs of our consumers;\n\nValid Driver’s license and car to be able travel throughout the state of New Jersey\n\nBenefits of working with FRN\nFull Time
