Thank you for expressing your interest in working with Fleet Healthcare (FHC). Prior to you commencing work with us we require the following documents and information.

Below is a list of documents that we require from you to remain compliant with the companies and agencies that support our clients. Items that are asterisked are essential. Items that aren’t asterisked are not essential, however, if not provided may prohibit you from treating certain clients.

• Superannuation Details (Fund Name, USI, and Membership Number) - only required if operating as a sole trader

• NDIS Worker Check (NDISWC)

Payment Information:

Prior to commencing work with Fleet Healthcare we are required to confirm payment arrangements with you. Working with Fleet Healthcare you have the option of being paid the FHC rates or setting your own rates. Please review the below information and advise which option in your application you would like to proceed below:

• FHC rates:

  If you are operating as a sole trader through your ABN, at this link you will find the "FHC Physiotherapist Payment List for Sole Traders"

  if you are operating as a company through your ABN, at this link you will find the "FHC Physiotherapist Payment List for Companies"

  Did you know that your ABN structure could affect how much you take home?

  If you operate through a company ABN, superannuation is not payable on top of your service fees - meaning the full rate (inclusive of what would otherwise be paid as super) goes directly to you. This can result in a higher amount in your pocket compared to operating as a sole trader, where superannuation is paid separately in addition to your service fees. Not sure which structure applies to you, or thinking about making a change? We'd recommend speaking with your accountant or financial adviser to find out what works best for your situation.

• Setting your own rates: In the "Application" section of this webpage you'll find a field where you can enter your preferred rates for the services you provide. We'll take a look and get back to you to let you know if we're able to make it work!

Service Area:

With Fleet Healthcare, you have the option to choose your Local Government Areas (LGA) within Sydney. Tailor your service locations to fit your preferences and convenience. Please open this link to reference when selecting your LGA: Sydney LGA Map. You will be required to make a selection in the application process.

Availability:

Working with Fleet Healthcare we offer great flexibility in allowing you to determine your own schedule and work when you want.

In the application, you will be asked to select which option you would prefer:

• Create set availability with Fleet Healthcare - please advise days / hours
• Be contacted when appointments become available in your service area
