Forestdale’s mission is “to ensure that children and families have the fundamental assets they need to thrive and live independently.”\nJob Summary:\nFamily Treatment and Rehabilitation Case Planner is responsible for working with families who are experiencing circumstances that threaten the safety and well-being of their children. Families that referred to the Family Treatment and Rehabilitation Program are currently experiencing mental health and/or substance abuse issues which place the children at risk. These identified family member can be either the parents and/or the child. The role of the Family Support Case Planner is to work with families to support, help strengthen and promote healthy growth and development within the family.\nMajor Responsibilities/Activities:\n Visit families in their homes, communities or at the agency as indicated by the Phase of Treatment -Initial, Baseline or Stabilization. The visits ranges from a minimum of 3 times per week (Initial Phase of Treatment); or 1 time per week (Baseline Phase of Treatment); or two times a month (Stabilization Phase of Treatment).\nProvide individual, family and group counseling, as needed.\nAdvocate for clients in their effort to secure services, such as public assistance, health insurance and immigration issues.\nCoordinate and maintain contact with workers at the Administration for Children’s Services\nMaintain contact with schools, mental health facilities/therapist and any other service providers.\nParticipate in Clinical Diagnostic Team Meetings, staff meetings, in-service trainings, and outside trainings for professional development\nDocument all case activity using the systems of record - N.Y.S. Connections and the N.Y.C. PROMIS\nAssist in emergency situations, as needed\nResponsible for ensuring compliance with all agency policies, procedures and governmental regulations\nOther duties as assigned.\nMinimum Requirements:\nBachelor or Master’s Degree in Social Work or a related credential from an accredited college or university.\nAt least1- 2 years of experience in case management, counseling and advocacy services.\nSome knowledge of the child welfare system, preferable.\nStrong organizational, written, and verbal communication skills.\nAbility to accommodate a flexible work schedule.\nBi-lingual ability preferred.\nSalary Range: \nBachelors - $29.12/hr - $31.04/hr\nBachelors (Bilingual)- $30.22/hr - $32.15/hr\nMasters - $30.22/hr - $33.24/hr\nMasters (Bilingual) - $31.59/hr - $34.34/hr\nMasters (Bilingual & License) $34.34/hr - $37.09/hr\nAdditional Information:\nForestdale Inc. is an equal opportunity employer that is proudly committed to becoming an anti-racist and multicultural organization that stands up for justice and equity.
