And you’ll find a place where your work changes lives - including your own.\nFraser is seeking a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will oversee the day-to-day operations of a group home while providing leadership, coaching, and training to staff. You will ensure compliance with Fraser policies and 245D regulations, support continuous quality improvement efforts, and help maintain a safe, supportive environment for both staff and the individuals we serve.\nThis hands-on leadership position includes working directly with individuals living in our group homes to support their personal goals, promote independence, and assist with daily living activities and personal care needs as required. A portion of your weekly schedule will be dedicated to providing direct support services.\nResponsibilities:\nProvide on-site supervision and leadership to staff at the group home or residential location.\n\nEnsure compliance with 245D regulations and Fraser policies.\n\nSupport active treatment service development and implementation.\n\nLead continuous quality improvement initiatives at the location.\n\nAssure necessary staff coverage, and recommend staffing, to meet the needs of the clients served and the budgetary parameters of Fraser.\nServe as a primary point of contact for clients, families, and community partners.\n\nMonitor and maintain accurate client and staff records, including documentation required by regulations.\n\nParticipate in emergency response and preparedness activities as needed.\n\nCoordinate the activities of daily living including personal care, hygiene, and grooming.\nCoordinate and/or complete all household repairs and other upkeep. \nAssure required documentation for all areas of client care are completed as required, including but not limited to Health Progress Notes, appointment forms, narrative notes, and data collection. \nCoordinate admissions, discharges and grievances, in collaboration with the Assistant Director and the Director. \nCollaborate with medical professionals to coordinate health and medical needs for each client served. \nQualifications:\nCandidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.\nHS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience\nBA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities \nAssociate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities \nFraser Offers Benefits for Full-time Employees (30+ hours per week):\nUKG Wallet - Eligible employees can access earned wages before payday for added financial flexibility\nMedical, dental and vision insurance\nHealth Savings Account (HSA) and Flexible Spending Account (FSA)\nEmployee Assistance Plan (EAP)\nLife, AD&D and Voluntary Life Insurance\nLong-Term Disability, Accident Insurance, Critical Illness Insurance and Hospital Indemnity Insurance\nPet Insurance\n403(b) Retirement Plan with Company Match\nWork-Life Balance; 5 weeks of paid time off annually (18 days PTO + 9 Paid Holidays) \nOpportunities for community involvement in Fraser-organized events\nCareer growth opportunities\nEmployee Referral Bonuses\nLocation, Schedule & Pay:\nLocation: Bloomington, MN \nPlymouth, MN \n\nThis position is full-time Monday-Friday, 40 hours/weekPeriodic weekend on-call shifts \n\nThe starting pay range for this role is $21.50 - $23.50 with an additional 2-dollar differential per hour depending on up qualifications and location \n$1,000 Hiring Bonus for external applicants \nWhy Join Fraser?\nMeaningful Impact Help individuals and families lead more connected, independent, and fulfilling lives.
