We know our services change lives; if that is something that you want to be a part of, we need you!\nWe have an immediate need for a full-time Patient Coordinator to join our Case Management team.\nThe Patient Coordinator plays a vital role in ensuring effective communication and support between our healthcare team and our patients.\nResponsibilities\nThe Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources. \nAccompany patients to their benefit renewal appointments to advocate for the patient and the program.\nVisit patient homes and update patient care. \nEffectively communicate and provide information to patients, caregivers, and physicians. \nUnderstand HIPAA and maintain confidentiality.\nDisplay a warm and friendly attitude.\nMinimum Qualifications\nStrong work ethic and relationship building skills.\nSelf-motivated with the ability to manage time appropriately.\nStrong communication skills inside of the office and outside.\nExperience with MS Office products (Outlook, Word, Excel, etc.)\nMinimum associate level education preferred.
